병원 간호사의 환자안전관리 활동 영향요인

국문 초록

목적 본 연구는 일개 대학병원 간호사의 환자안전관리 중요성 인식, 간호근무환경, 환자안전역량과 환자안전관리 활동 정도를 조사하고, 환자안전관리 활동에 영향을 미치는 요인을 파악하고자 시행되었다. 방법 자료 수집 기간은 2021년 9월 1일부터 2021년 10월 30일까지이며, J도 소재 W대학병원 간호사 195명을 대상으로 실시하였다. 자료 분석은 SPSS/WIN 26.0 프로그램을 이용하여 기술통계, one way-ANOVA, Pearson’s correlation coefficients와 위계적 선형 회귀분석을 실시하였다. 결과 본 연구에서 대상자의 환자안전관리 중요성 인식은 3.78점(5점 만점), 간호근무환경은 3.04점(5점 만점), 환자안전역량은 3.83점(5점 만점), 환자안전관리 활동은 4.40점(5점 만점)으로 나타났다. 환자안전관리 중요성 인식, 간호근무환경, 환자안전역량과 환자안전관리 활동의 상관관계를 분석한 결과, 환자안전관리 중요성 인식은 간호근무환경(r=.44, p<.001), 환자안전역량(r=.61, p<.001), 환자안전관리 활동(r=.35, p<.001)과 유의한 양의 상관관계를 보였다. 간호근무환경은 환자안전역량(r=.28, p<.001)과 유의한 양의 상관관계를 보였으며, 환자안전역량은 환자안전관리 활동(r=.59, p<.001)과 유의한 양의 상관관계가 있는 것으로 나타났다. 위계적 회귀분석을 통해 대상자의 환자안전관리 활동에 영향을 미치는 요인은 총 임상경력 15년 이상(β=-.24, p=.027), 환자안전역량(β=.63, p<.001)으로 이 모형의 설명력은 약 36%로 나타났다(F=14.89, p<.001). 결론 대학병원 간호사의 환자안전관리 활동을 향상시키기 위해서는 지속적으로 임상에 재직할 수 있도록 다양한 제도적 장치가 마련되어야 하며, 환자안전역량을 높이기 위한 체계적인 교육 및 훈련 프로그램이 개발되어야 할 것이다. 또한, 간호사의 환자안전역량을 뒷받침 할 수 있도록 충분한 행정적 지원, 근거기반의 간호지침들이 필요할 것이다.

영문 초록

Objectives The purpose of this study is to survey the perception of importance of patient safety management, nursing work environment, patient safety competence and patient safety management activities focusing on one university hospital nurses in order to identify the factors that influence patient safety manage activities. Methods The participants were 195 nurses from one university hospital located in Korean J province. Data were collected between September 1, 2021 to October 30, 2021 from a self-administered questionnaire and were analyzed using the t-test, One-way ANOVA, Pearson correlation coefficients and hierarchical regression analysis with the SPSS 26.0 program. Results The correlation among importance perception of patient safety management, nursing work environment, patient safety competence, and patient safety management activities showed a significant positive correlation between the degree of the importance perception of patient safety management, nursing work environment, patient safety competence, and patient safety management activities. Nursing work environment showed a significant positive correlation with patient safety competence while patient safety competence had a significant positive correlation with patient safety management activities. The factors affecting the patient safety management activities which were identified using hierarchical regression analysis were more than 15 years of total clinical experience, patient safety competence. The explanatory power of this model was 36% (F=14.89, p<.001). Conclusions In order to improve the patient safety management activities of nurses, various institutional devices should be prepared so that they can continue to work in hospital, and systematic education and training programs should be developed to enhance patient safety competency. In addition, sufficient administrative support and evidence-based nursing guidelines will be needed to support nurses patient safety competency.
