국군간호사관생도의 임상실습 스트레스, 의사소통 능력, 발언행동, 침묵행동 및침묵동기가 임상수행능력에 미치는 영향

국문 초록

본 연구는 국군간호사관생도의 임상실습 스트레스, 의사소통 능력, 발언행동, 침묵행동 및 침묵동기가 임상수행능력에 미치는 영향을 검증하여 향후 임상실습을 설계하고 운영함에 있어 보다 전략적인 방안을 제시하는 데 그 목적이 있다. 연구 대상 자는 임상실습 교과목을 수강신청하고 최소 1학기(15주) 이상 임상실습 경험이 있는 3,4학년 150명으로 2018년 7월 28일부터 8월 30일까지 자료를 수집하였다. 수집된 자료는 SPSS 23.0 통계프로그램을 사용하여 분석하였다. 본 연구 대상자의 임상수행 능력에 대하여 의사소통 능력(r=.42, p<.001), 발언행동(r=.30, p<.001)은 양의 상관관 계를 보였으며, 침묵행동(r=-.24, p=.002), 침묵동기(r=-.18, p=.025), 체념적 침묵동기 (r=-.21, p=.008)는 음의 상관관계가 나타났다. 단계적 회귀분석 결과, 임상수행능력에 영향을 미치는 요인은 의사소통 능력과 발언행동이었다. 임상수행능력에 대한 설명력은 의사소통 능력이 17%였고, 발언행동이 추가되면서 3%의 설명력이 증가 되어 총 20%의 설명력을 나타냈다. 이 연구를 통해 초급간부를 양성하는 특수목적의 기관에서 리더로 성장하는 국군간호사관생도들에게는 의사소통 능력 이외에도 발언행동을 임상수행능력의 지표로 고려해 볼 필요가 있다. 추후 임상수행능력 향상을 위하여 환자안전 및 간호오류와 관련된 문제해결을 위한 발언행동 관련 프로그램 개발을 제언한다.

영문 초록

This study provides a more strategic approach to designing and operating future clinical practices by verifying the impact of the clinical practice stress, communication, voice behavior, silent behavior, and silent motivation of the Korea Armed Forces Nursing Academy Cadets on clinical performance. The study participants applied for clinical practice classes and collected 150 students from July 28 to August 30, 2018 with clinical practice experience for at least 15 weeks in the first semester. Data collected were analyzed using the SPSS 23.0 statistical program. The clinical performance of this study were positively correlated with the ability of communication (r=.42, p<.001) and voice behavior (r=.30, p<.001). However, clinical performance has a negative correlation with silent behavior (r=-.24, p=.002), silence motivation (r=-.18, p=.025), and acquiescent silence motivation (r=-.21, p=.008). As a result of the stepwise regression analysis, the factors affecting clinical performance were communication skills and voice behaviors. Explanatory power of clinical performance was 17% in ability to communicate, and the addition of voice behavior increased 3% to a total of 20%. In addition to communication, the study needs to consider voice behaviors as an indicator of clinical performance for cadets who grow to be leaders in special-purpose institutions that cultivate first-level officers. To improve clinical practice later on, it is proposed to develop a program related to voice behaviors to solve problems related to patient safety and nursing errors.
