틱 장애를 가진 아동의 치료놀이 적용에 대한 임상적 사례연구

국문 초록

목적 본 연구는 치료놀이를 적용하여 틱 장애 아동과 어머니의 상호작용 증진 효과, 아동의 틱 증상 감소 효과를 검증하기 위함이다. 방법 본 연구는 치료놀이를 통해 틱 장애 아동과 어머니의 상호작용의 변화 요인과 양상을 심도 있게 이해하기 위한 단일사례 연구 방법으로 진행되었고, 현상학적 관점의 임상적 사례연구 이다. 본 연구의 치료놀이 프로그램은 총 20회기로 구성하였다. 결과 본 연구의 주요 결과는 다음과 같다. 첫째, 틱 장애 아동과 어머니의 치료놀이 과정은 어떠한가에서 치료놀이 실시 후 회기별로 살펴본 결과 틱 장애 아동과 어머니는 애착이 증진되고 상호작용 행동이 점차 증가하여 긍정적으로 변화되었음이 입증되었다. 또한 치료놀이 참여 전⋅후 MIM을 통한 분석 결과 틱 장애 아동과 어머니의 상호작용이 긍정적으로 증진됨이 나타났으며 치료놀이 효과가 입증되었다. 둘째, 틱 장애 아동과 어머니의 변화에 대한 치료적 요소는 아동의 안정감을 확고히 하고 저각성을 조절한 것, 양육자와 아동이 애착을 향상한 것, 그리고 양육자인 어머니가 치료사와 상담 하면서 자신을 성찰 할 수 있었고 아동과의 상호작용 변화 필요성을 인식하게 된 것이라고 할 수 있다. 또한 틱 장애 아동과 어머니의 상호작용을 통하여 치료놀이 적용 후 아동의 틱 증상이 현저히 줄었음을 알 수 있다. 결론 본 연구의 결과는 치료놀이가 틱 장애 아동과 어머니의 긍정적 상호작용 증진과 아동의 틱 증상 감소에 효과가 있음을 입증하였다.

영문 초록

Objectives The purpose of this study is to apply the Theraplay program to the child with tic disorder to trigger the interactions between these child and their parents, with a view to verify the effect of such play programs on treating tic disorder in child Methods This study was conducted as a case study with a view to obtainc an in-depth understanding of the causes for the changes in tic disorder through Theraplay and it is a clinical case study from a phenomenological point of view. The Theraplay of this study was provided over 20 sessions. Results The key findings of this study were as follows; First, after the execution of Theraplay, the child and their mother appeared to form a bond after each session, with a gradual increase in interactions, which proved that there were positive changes. Second, the comparison of before and after the participation in the Theraplay games, the child with tic disorder and their mothers saw a positive improvement in their interaction with each other, and the persistence of the effect was also supported. Third, the mothers used the interactions to form affection based on a sense of stability with their child, and these mothers reflected upon themselves to recognize the need for a change in their interactions with their child. Fourth, after the application of the Theraplay program regarding the interactions between the child with tic disorder and their mothers, it was shown that the tic symptoms of the child decreased significantly. Conclusions The findings of this study verified that the Theraplay program had a positive impact on the promotion of positive interactions between the child with tic disorder and their mothers, as well as the reduction of the tic symptoms of the said child
