국문 초록

의학교육의 목표는 의대생이 바람직한 의사 역량에 갖추어 정확한 진단과 진료를 독자적으로 할 수 있도록 교육과 훈련을 시키는 것이다. 의대에서는 의사국가고시 기준에 맞춘 실기 시험을 포함하여 수시로 학생을 학습 도달 정도를 평가한다. 실기 시 험은 임상수행능력평가시험과 객관구조화진료시험을 의미하며 의대생에게 요구하는 지식, 술기, 태도 역량을 두루 검증하는 시험이다. 실기 시험은 의학교육 과정이 거의 마무리 되어 가는 시점에 시행되는 시험으로 통합적 역량을 측정한다. 평가의 실시시점이 후반부에 있기 때문에 이전 평가들에서는 드러나지 않았던 의사소통의 문제점이 새로이 발견되기도 하다. 의대는 학습 목표에 도달하지 못한 학생을 유급시키는 제도가 있다. 유급 제도의 목적은 의료의 질을 안정적으로 유지한다는 데에 있다. 그러나 유급 제도는 몇 가지 문제점을 가지고 있으며 의료의 질을 유지하기 위한 목적으로 유급보다는 재교육을 통해 더 우수한 의사를 배출할 필요가 있다. 특히 환자중심진료를 표방한 진료 현장에서 여러 역량과 상호작용하면서 나타나는 의사소통역량은 의대생이 갖추어야 할 역량임에도 불구하고 이에 부족함을 보이는 학생을 풍부한 임상 경험을 가진 교수가 지도하는 것이 중요하다. 학생을 면밀히 파악하여 학생 개개인에게 적절한 피드백을 제공한다면 학생은 자신의 문제를 인지하고 기주도적으로 의학적 처치를 구조화할 수 있게 될 것이다. 부족한 역량을 가진 학생은 재교육을 통해 낙제와 유급이 아닌 숙련된 의사로 성장하도록 기회를 제공할 수 있다.

영문 초록

The goal of medical education is to lead medical students to equip with good physician capacity to make accurate diagnosis and treatment independently. The medical school regularly assess the degree of student’s achievement, including the clinical skills assessments that conform to the Korean national medical licensing examination. Clinical skills assessments are called a clinical practice examination and an objective structure Clinical examination, and these are tests to verify the medical knowledge, skills, and attitude required for medical students. These tests are almost conducted at the end of the course. It is also the time point at which communication problem that was not revealed in previous evaluations are newly discovered. Medical schools have a system to be held back students who fail to reach a learning goal. The purpose of the flunk system is to stabilize the quality of medical care. However, this system has several problems, so it needs to study other ways making good doctors throughout remediation rather than flunk for maintaining medical quality. In particular, it is important that the professors with rich clinical experience lead the students who has the lack in their communication capacity despite those should have the ability to interact with various competencies on the patients-centered clinical field. Students will enable to recognize their problems and structure their own medical initiatives by providing appropriate feedback to each student. Students with insufficient competencies mat be able to opportunities to grow as skilled physicians, not failing and flunk through remediation.
