경피신경전기자극이 수술후 통증에 미치는 효과에 대한 메타분석

국문 초록

본 연구의 목적은 통증조절을 위해 수술후 경피신경전기자극(TENS)을 적용한 연구를 메타분석하여 경피신경전기자극의 사용에 관한 과학적 근거를 마련하기 위함이다. 2000-2010년까지의 무작위 대조군 전후 실험설계연구 16편의 연구물로부터 산출된 183개의 효과크기를 SPSS 17.0프로그램을 이용하여 경피신경전기자극이 통증에 미치는 효과, 효과적인 수술부위, 적용기간, 적용방법을 메타분석 하였다. 그 결과 경피신경전기자극의 전체 효과크기를 산출한 결과 중간정도의 효과크기를 나타내었으며 전체 중재에 따라 종속변수별로 분석한 결과 통증, 진통제 사용량과 활력징후에는 큰 효과크기를 나타내었으나 폐기능은 작은 효과크기를 나타내었다. 중재기간별 효과크기는 중재기간 1일이 큰 효과크기를 나타내었으며 수술부위는 복부, 어깨, 척추와 편도 수술에서 큰 효과크기를 나타내었다. 경피신경전기자극 적용방법은 지속적, 저빈도, 편안한 강도로 했을 때 효과크기가 큰 것으로 나타났다. 본 연구의 결과 경피신경전기자극이 수술 후 통증을 감소시키고 진통제 사용량을 줄이는데 효과적인 것으로 밝혀졌으므로 이를 바탕으로 표준화된 경피신경전기자극 간호중재 프로토콜과 프로그램을 개발하여 임상과 지역에서 적용할 필요가 있다고 본다.

영문 초록

Purpose: The purpose of this study was to analyze the effect of transcutaneous electrical nerve stimulation on the postoperative pain. Methods: Meta-analysis was done with 16 published studies, article and research papers that had been published from 2000 to 2010, The selected studies had a randomized control group and the researchers identified 16 studies with 183 research variables Result: Overall mean effect size of the TENS was moderate level. The mean effect size of the postoperative pain variables, analgesics consumption variables, vital signs and the results showed effect large In the mean effect size of TENS application period one day was showed effect large. The site of surgery in abdomen, shoulder and spine was showed effect large. Most effective on the TENS intervention method of the continued TENS, lower frequency comfortable intensity stimulation was showed effect large. Conclusion: TENS was showed effectively on postoperative pain and analgesics consumption. therefore development of standardized protocol and program of TENS nursing intervention then applied in clinical and community settings.
