집단회상요법이 입원치매노인의 우울 및 문제행동에 미치는 효과

국문 초록

본 연구는 집단 회상요법이 입원 치매노인의 우울 및 문제행동에 미치는 효과를 규명한 것으로 강원도 C시 E병원에 치매로 진단받고 입원한 환자 중에서 연구 조건을 만족하는 19명을 실험군으로, 비슷한 치료환경이면서 지리적으로 떨어져 있는 K병원에 치매로 입원 중인 노인 21명을 대조군으로 선정하여 집단회상요법을 적용하여 그 효과를 검증한 연구이다. 연구결과는 첫째, 집단회상요법은 치매노인의 우울(t=-6.878, p<.001)과 문제행동(t=-3.916, p<.001)을 감소시키는데 효과적이다. 둘째, 치매노인의 우울과 문제행동 간에는 상관관계가 있다. 본 연구는 노인 우울에 대한 상반된 연구 결과를 재검증함으로써 회상요법이 우울의 감소에 효과적임을 확인하고 치매노인을 대상으로 한 연구가 드문 실정에서 선행 연구를 반복연구 함으로써 회상요법이 치매노인에게 우울중재 효과가 있음을 확인하였다는데 그 의의가 있다. 또한 간호 실무 면에서 회상요법이 치매노인에게 안전하고도 적용이 용이하여 대상자나 치료자 모두에게 있어 실용성, 유용성이 높은 중재기법이라는 것을 확인하였다. 이상의 연구 결과를 근거로 집단회상요법은 입원치매노인의 정서적, 행동적 문제에 효과적인 간호중재 프로그램으로서 간호 실무에 활용될 수 있을 것이다.

영문 초록

Purpose : This study introduced group reminiscence therapy as a nursing intervention program and aimed at reducing depression and problem behavior in the elderly with dementia. Methods : This study was designed with the non-equivalent control group pretest-posttest design. The experimental group included 19 elderly people with dementia admitted to a hospital in C city, Kangwon province and the control group had 21 elderly people with dementia in similar treatment environment. We measured depression and behavior problem with Geriatric Depression Scale Short Form - Korea (GDSSF-K) and Memory & Behavior Problem Checklist (MBPC). Data were analyzed with chi-square test, fisher’s exact probability test and t-test. Results : Depression score of the experimental group had significantly lower than that of the control group. Problem behavior score of the experimental group had significantly lower than that of the control group. A clear correlation was found between depression and problem behavior. Conclusion : The results of this study confirmed that the group reminiscence therapy is effective in reducing depression and problem behavior of the elderly with dementia hospitalized. Therefore, the therapy should be utilized in clinical practice as an effective nursing intervention for the elderly with dementia.
