Compliance Comparison of the Volumetric Modulated Arc Therapy QA by Using 2D and 3D Array Detectors

국문 초록
표적체적에는 충분한 치료선량을 조사하면서 주변 정상조직에는 최소한의 선량을 주기 위해 최근 다양한 기술들이 개발되고 있다. 본 연구에서는 가장 보편적인 치료방법 중 하나인 부피적 세기조절 회전방사선치료의 QA 시 어떤 기기가 더 적합한지 2D Array Detector와 3D Array Detector를 이용하여 평가한다. 30명의 환자를 대상으로 하여 MONACO 3.3 (Elekta Medical System, Sweden)을 이용하여 치료계획을 세운다. 2개의 QA기기를 테이블 위에 올려놓고, Isocenter에 정렬시킨 후 방사선량을 전달하며, 실제 치료계획의 선량과 QA장비를 통한 선량 비교 프로그램으로 SNC Patient 6.1 (Sun Nuclear, USA)를 사용하여 정확도를 측정한다. 2D Array Detector로 실시할 때 18개가 95% 이하, 2D Array Detector(Gantry angle을 0로 모두 변환한 치료계획)로 실시할 때 모든 값에서 95% 이상, 3D Array Detector로 실시할 때 1개를 제외하고 95% 이상을 나타낸다. 최대값은 99.2, 100, 99.2, 최솟값은 87.4, 95.3, 93.7, 평균값은 93.99±0.65, 98.96±0.22, 97.86±0.24으로 2D Array Detector, 2D Array Detector(Gantry angle을 0로 모두 변환한 치료계획), 3D Array Detector에서 각각 나타난다. 3가지 QA 기기 중 2D Array Detector(Gantry angle을 0로 모두 변환한 치료계획)가 가장 높은 정확도로 나타났지만, Gantry 앵글에 따른 MLC Speed등을 고려하지 못한다. 2D Array Detector는 수직인 빔을 측정하기 위한 구조로 이루어져 있어 여러 각도에서 들어오는 빔을 다이오드가 구성하는데 어려움이 있는 반면, 3D Array Detector는 입체표면으로 이루어져 있어 여러 각도에서 오는 빔을 다이오드가 받아들일 수 있기 때문에 더 높은 정확도를 나타낸다. 따라서 부피적 세기조절 회전 방사선치료 QA로 모든 성분을 고려하고 정확도가 더 높은 3D Array Detector가 가장 적합하다.
