국문 초록
본 연구에서는 복부 검사 시 방사선 피폭을 최소로 줄이기 위해 선량에 따라 얻어지는 SECT와 DECT의 영상을 정략적으로 비교평가 하였다.
GE Discovery 750 HD CT와 Acrylic phantom, Unfors Xi Ion chamber와 CT Abdomen Phantom을 이용하였다. 선량을 평가하기 위한 SECT 스캔 조건은 120 kVp 와 Axial mode로 영상을 획득하였다. CTDIvol은 25.13 mGy에서 8.96 mGy으로 5개 단계로 구분하여 총 7회 측정하였다. DECT 스캔 조건은 80/140 kVp를 사용하고, 나머지 스캔 조건은 SECT와 동일하게 설정하였다. 복부팬텀의 SECT 스캔 조건은 120 kVp, Helical Mode로 영상을 획득하였다. CTDIvol 은 25.53 mGy에서 9.05 mGy으로 5개 단계로 구분하여 총 10회 측정하였다. DECT 스캔 조건은 40 keV 에서 100 keV 까지 10 keV 별로 7개 단계로 구분하여 영상을 획득하였다. 나머지 scan조건은 SECT와 동일하게 설정 하였다. SECT와 DECT에서 CTDIvol별로 선형 회귀 분석을 한 결과 R² 값이 각각 0.9586, 1로 직선성이 선형적으로 반영되었다. SNR평가에서 CTDIvol 9.05 mGy의 DECT 70keV 가 CTDIvol 12.92 mGy의 SECT 120kVp 보다 Liver, Stomach, Kidney, Psoas muscle에서 유의한 차이를 보였다(p<0.05). CNR 평가에서 CTDIvol 9.05 mGy의 DECT 70 keV가 CTDIvol 12.92 mGy의 SECT 120kVp보다 Liver, Stomach, Kidney, Psoas muscle에서 유의한 차이를 보였다(p<0.05). 선량 별 장기들의 SNR, CNR 평가에서 선량을 30% 감소시킨 DECT 70 keV 영상이 SECT 120 kVp 영상보다 우수하였다. 따라서 복부 검사 시 DECT를 사용하면 선량을 30% 감소시킬 뿐만 아니라 SECT보다 진단적 가치가 높은 영상을 구현할 것이라고 사료된다.
