국문 초록
컴퓨터단층촬영은(이하 CT) X선관과 검출기가 인체를 중심으로 회전하여 영상을 만들어내는 단층촬영법이다. 지난 20여 년간 CT는 우수한 공간 및 대조도 분해능으로 인한 검사의 정확성과 검사방법의 용이성으로 대표적인 디지털 영상 검사로서 유용성이 인정되어 왔다. 혁신적인 CT 기술 발전으로 인한 진단적 가치의 향상으로 검사건수가 크게 증가하였으며 전체 방사선 검사에서 CT검사의 비중이 크게 증가하고 있다. 본 논문에서는 환자가 iso-center에 위치하지 못하여 중심선속에서 벗어났을 때의 선량과 iso-center에 위치한 환자 피폭선량의 차이에 대한 의문점으로 연구가 시작 되었다. 또한 CTDI abdomen phantom과 CTDI head phantom 그리고 선량계를 이용하여 spiral scan 방식으로 진행되는 CT검사에서 off-centering에 따른 선량변화와 영상에 미치는 영향에 대해 알아보고자 하였다.
본 연구 장비로는 Alexion 16 channel CT (Toshiba, Tokyo, Japan)(a)기기를 사용하였다. Phantom으로는 32 cm CTDI abdomen phantom(b)과 16 cm CTDI head phantom(c)을 사용하였고. 마지막으로 선량값을 측정하기 위해 Unfors xi probe (Unfors Instruments AB, Billdal, Sweden)을 사용하였다.
그 결과 Head phantom에서 선량은 상방 3 cm과 하방 3 cm에서는 증가하였지만 상방 6 cm와 하방 6 cm에서는 감소하였다. Abdomen phantom의 선량은 iso-center에서 더 많이 이동할수록 감소하였다. Head phantom 스캔에서 CTDIvol (52.90 mGy), DLP (179.60 mGy·cm)로 측정되었고, abdomen phantom의 10 range 스캔에서 CTDIvol (12.80 mGy), DLP (59.10 mGy·cm), 50 range에서 CTDIvol (12.90 mGy), DLP(111.20 mGy·cm)로 측정되었다. CT 검사에서 iso-center에서 더 많이 벗어날수록 영상의 노이즈가 증가하였고, 균일도는 감소되었다.
결론적으로 CT검사에서 iso-center에서 벗어날수록 노이즈가 증가하고 균일도가 감소되므로 영상의 질이 저하된다. 따라서 피폭선량과 영상의 질을 고려하여 iso-center에 정확히 환자를 위치하여 검사해야 한다.
