중환자실 퇴실 환자를 위한 전환간호 프로그램 개발 및 적용 효과

국문 초록

본 연구는 중환자실에서 일반 병동으로 전실하는 환자를 대상으로 간호의 연속성 유지를 위한 전환간호 프로그램을 개발하고, 프로그램의 효과를 파악하였다. 전환간호 프로그램은 문헌고찰과 전문가 타당도 및 임상타당도 평가를 통해 개발되었다. 프로그램의 적용 효과를 파악하기 위해 서울 소재 일개 상급종합병원 내과중환자실에 입실하여 치료를 받은 후 신체 증상이 호전되어 일반병동으로 전실한 환자 68명(실험군 33명, 대조군 35명)을 대상으로 프로그램을 적용하였으며, 중환자실 재입실 건수, 예기치 않은 사망 건수, 조기대응팀 협진 건수 및 전환간호만족도를 포함한 자료를 수집하였다. Windows SPSS 21 프로그램으로 실수, 백분율, 평균과 표준편차, t-검정 또는 Mann-Whitney U 검정, 카이제곱검정 또는 Fisher 정확검정을 이용하여 수집된 자료를 분석한 결과, 환자의 중환자실 재입실 건수, 예기치 못한 사망 건수, 신속대응팀 협진 건수에는 유의한 차이가 없었으나 전환간호에 대한 만족도는 높은 것으로 나타났다. 이와 같은 본 연구의 결과를 토대로 좀 더 장기간 전환간호 프로그램을 적용하거나 개별 환자의 특성을 고려한 전환간호 프로그램의 개발이 필요하다.

영문 초록

This study aimed to develop the transitional care program for continuity of care for patients in intensive care units (ICUs) and to evaluate its effects including the number of readmissions to ICU, the unexpected deaths, the number of rapid response team consultancy, and patient’s satisfaction with care. The program was developed through a literature review and validation of an expert group. The developed program was tested with 33 patients of the experimental group and 35 patients of the control group in a medical intensive care unit in a tertiary hospital. Data were collected from February 2014 to May 2014 and analyzed by descriptive statistics, t-test, Mann-Whitey test, chi-square test, and Fisher‘s exact test using the SPSS/Win statistical program. Satisfaction with transitional care of experimental group was significantly higher than that of control group. There were no significant differences in the number of readmissions to ICU, the unexpected deaths, and the number of rapid response team consultancy. The developed transitional care programs are proved to be partially effective to provide the continuity of care. To identify more positive effects of the transitional care, application of this program to clinical practices during longer period can be recommended.
