비열등성 임상시험의 비열등성 한계 설정에 대한 체계적 문헌고찰과 메타분석

국문 초록

임상시험은 인간을 대상으로 약물의 효과 및 안정성을 평가하기 위해 계획된 실험을 의미한다. 그 중 비열등성 임상시험은 시험약물의 유효성이 활성대조군 혹은 기존의 치료방법에 비해 사전에 정의된 비열등성 한계보다 임상적으로 나쁘지 않다는 것을 입증하는 데에 그 목적이 있다. 본 연구에서는 미국 국립 의학 도서관에서 제공하는 검색엔진 펍메드(PubMed)를 통해 2009년 1월부터 2010년 12월까지 수행된 비열등성 임상시험에 대한 체계적 문헌고찰을 실시하였다. 또 유사 선행 연구인 Anne et al.(2006)의 연구와 비교 분석을 실시한 결과, 총 101편의 연구논문 중 비열등성 한계에 대한 언급이 없는 논문 9편(8.9%), 계획서 순응군 분석을 실시하지 않은 논문 53편(52.5%) 등이 발견되어 비열등성 임상시험 보고 시 적절한 통계학적 방법과 표준화된 용어 사용의 필요성을 확인하였다. 또 주 평가변수로 당화혈색소(HbA1c)를 고려한 8편의 연구논문으로 메타분석을 실시해 전반적인 비열등성 한계 크기를 추정한 결과, 랜덤효과모형을 통해 0.313으로 추정되었으며 그에 대한 95% 신뢰구간은 (0.295-0.331)로 나타났다. 출판편향 오차를 보정하기 위해 Duval and Tweedie의 절삭과 채움 방법을 적용한 결과, 그 크기는 0.309로 추정되었으며 그에 대한 95% 신뢰구간은 (0.291-0.327)로 보정되었다.

영문 초록

A clinical trial (CT) is defined as a prospective study comparing the effect and value of intervention against control in human beings. Among clinical trials, non-inferiority (NI) trials are intended to show that the effect of a new treatment is not worse than that of an active control by more than a pre-specified margin. For systematic review, we searched medical articles of randomized controlled trials of non-inferiority between January 1, 2009, and December 31, 2010 using PubMed provided by U.S. National Library of Medicine. Also, comparison with former study performed by Anne et al. (2006) was summarized. As results, among a total of 101 articles there were deficiencies in reporting of non inferiority trials such as absence of NI margin [9 articles (8.9%)], PP analysis not performed [53 articles (52.5%)] and so on. Therefore, adequate statistical methodologies and standard vocabulary were required to improve the quality of reporting NI trials. In order to estimate overall NI margin, meta analysis was performed with 8 articles whose primary endpoint is HbA1c. The estimated overall NI margin (95% confidence interval) was 0.313 (0.295-0.331) derived from random effects model, and 0.309 (0.291-0.327) adjusted by Duval and Tweedie’s trim and fill method for reducing publication bias error.
