민간의료보험이 노년층 의료서비스 이용에 미치는 영향: 만성질환 보유 여부에 따른 차이

국문 초록

본 논문은 의료서비스 이용 비중이 높은 만 65세 이상 노년층의 의료서비스 이용과 관련하여, 민간의료보험 가입이 외래서비스 이용횟수 및 입원서비스 이용일수에 미치는 영향을 분석하였다. 특히 노년층에 만연한 만성질환 보유 여부에 따라 이러한 영향이 어떻게 차이가 나는지에 대한 비교분석을 하였다. 이를 위해 한국복지패널의 2007년~2017년의 자료를 이용한 패널 포아송 회귀모형을 추정하였으며, 그 결과 민간의료보험 가입은 노년층의 외래서비스 이용횟수 및 입원서비스 이용일수 등 의료서비스 이용을 모두 뚜렷이 증가시키는 것으로 나타났다. 특히 만성질환을 앓고 있는 노년층에 비해 만성질환 없는 건강한 노년층에서 그러한 민간의료보험 가입으로 인한 영향의 크기가 훨씬 크게 나타났다. 이러한 결과는 민간의료보험이 노년층의 미충족의료서비스 수요를 충족시켜주는 긍정적 역할을 하지만, 상대적으로 의료서비스 수요가 적을 수밖에 없는 건강한 노인들의 의료서비스 이용을 더욱 크게 유인하는 비효율적 도덕적 해이의 존재 가능성을 시사한다. 급속한 고령화 속에서 중요성이 커지는 노령인구의 의료보건 문제와 관련하여, 이는 한정된 의료서비스 자원의 비효율적 배분을 결과하고 공적보험재정에도 불필요한 부담을 주게 되는 만큼, 노령인구 대상의 민간의료보험 시장에 대한 세심한 정책적 고려가 요구된다.

영문 초록

This paper analyzes the effect of private medical insurance subscription on the number of outpatient services and the days of hospitalization in relation to the use of medical services among the elderly over 65 years of age, who consume a high proportion of medical services. In particular, a comparative analysis was conducted on how these effects differ depending on whether or not they have chronic diseases that are prevalent in the elderly. To this end, the panel Poisson regression model was estimated using data from 2007 to 2017 of the Korean welfare panel study. Estimation results show that private medical insurance subscription significantly increased the use of medical services such as the number of outpatient services and the days of hospitalization by the elderly. In particular, compared to the elderly with chronic diseases, the influence of such private medical insurance was much larger in healthy elderly people without chronic disease. This supports the fact that private medical insurance has a positive role in satisfying the demand for unsatisfied medical services of the elderly, but there is a moral hazard that further encourages the use of medical services by healthy elderly people who have relatively low demand for medical services. This leads to inefficient allocation of limited medical service resources and an unnecessary burden on public insurance finances in relation to the medical and health problems of the elderly population, which is growing in importance amid the rapid aging population. Careful policy consideration is required for the private medical insurance market for the elderly.
