보건소 금연클리닉 금연성공자의 재흡연 관련요인

국문 초록

본 연구는 U시 5개 구군 전체 보건소 금연클리닉에 등록하여 6개월간 금연에 성공하여 퇴록한 전수 대상자인 2,046명 중 본 연구에 자발적으로 동의한 773명을 대상으로 최초 금연 성공 24개월 시점에서 재흡연 관련요인을 파악하기 위한 후향적 추적조사연구이다. 수집된 자료는 의사결정나무분석과 Kaplan-Meier 방법과 콕스비례위험모형(Cox proportional hazards model)을 이용하여 분석하였다. 연구결과 최초 금연 후 2년 이상 장기금연을 유지하는 대상자는 전체의 66.5%이었고, 대상자의 5.6%가 금연클리닉을 퇴록한 한 달 이내에 재흡연을 한 것으로 나타났다. 대상자의 평균 금연지속기간은 6개월 금연성공 퇴록 후 16.90개월 이었으며, 재흡연의 관련요인은 스트레스, 주관적 건강상태, 금연클리닉 당시 호기일산화탄소 농도로 나타났다. 금연지속기간과 재흡연의 변화를 생존분석해본 결과 최초로 금연에 성공한 후 12개월까지의 재흡연율이 높았으며, 특히 주관적 건강상태가 나쁘고, 스트레스수준이 8이상, 금연클리닉 등록 당시 호기일산화탄소 농도가 20ppm 이상인 대상자의 재흡연률이 높았다(p<0.05).

영문 초록

This research is the survey research based on a retrospective follow-up survey which was conducted on 773 persons, out of the population of 2,046, who had cessation smoking for 6 months and registered in the smoking cessation at the public health center in U city. Data were collected after 24th month from the first time cessation smoking. The collected data were analyzed using the decision-making tree analysis and Kaplan-Meier method survival analysis. The results showed that 66.5% of the subjects maintained a long-term nonsmoking state for more than 2 years from the first time cessation smoking, and 5.6% relapsed smoking with in one month from the time of getting out of the smoking cessation clinics. The subjects’ average nonsmoking duration was 16.9 months, and the factors for the relapse of smoking were stress, subjective health conditions, and breath carbon monoxide concentration at the time of smoking cessation clinics. The results of conducting survival analysis on the nonsmoking duration and changes in the relapse of smoking showed that the rate of relapse of smoking of the period from the time of first success in cessation smoking to the 12th month was high, the stress level was greater than 8, and the breath carbon monoxide concentration was greater than 20 ppm, the rate of relapse the subjects was high (p<0.05).
