알코올의존 입원 남성 환자의 감정표현불능증과 음주갈망

국문 초록

본 연구는 입원한 알코올의존 남성 환자의 감정표현불능증과 음주갈망의 정도를 확인하고 감정표현불능증과 음주갈망의 관계를 규명하기 위한 조사연구이며, 연구대상자는 정신과 전문병원의 알코올중독 병동에 입원한 남성 환자 173명이다. 연구 결과 입원 환자의 88.4%가 위험 음주의 수준이었고, 대상자의 감정표현불능증 정도가 높았다(60.8점). AUDIT-K 수준에 따른 감정표현 불능증은 집단 간에 유의한 차이가 없었으며, AUDIT-K 수준에 따른 음주갈망은 알코올사용장애 집단이 문제성 집단과 비문제성 집단보다 더 높았다. 감정표현불능증과 음주갈망 간에 유의한 부적 상관관계(r=-.28, p<0.001)가 있는 것으로 나타났다. 즉 감정표현불능증이 낮은 대상자가 음주갈망이 높은 것으로 나타났다. 따라서 본 연구는 알코올의존자의 감정표현불능증과 음주갈망과의 관련성 연구의 첫 시도로써, 향후 확대된 연구의 경험적 근거자료의 토대가 될 것이다. 또한 알코올의존 환자의 중재프로그램을 계획할 때, 부정적 정서와 함께 감정표현 불능증과 음주갈망에 대한 평가 및 대처방안 포함이 필요하다고 본다.

영문 초록

The purpose of this study is to identify the relationship of alexithymia and alcohol craving in male inpatients with alcohol dependence in an alcoholic ward. A total of 173 male alcoholic inpatients participated in this cross-sectional descriptive study. Alexithymia was assessed using the Korean version of 20-item Toronto alexithymia scale (TAS-20K) and alcohol craving was assessed using obsessive compulsive drinking scale (OCDS). Data were analyzed using one-way ANOVA, Pearson correlation coefficients with SPSS/WIN 18.0. Problematic group was 88.4% (over 12 by AUDIT-K score). There was no significant relation between alexithymia and AUDIT-K scores. Alcohol craving was different depending on the level of AUDIT-K scores. There was a negative correlation between alexithymia and alcohol craving. These results suggest that the assessment of alexithymia, negative mood, and alcohol craving should be performed in the diagnostic evaluation of alcoholic patients. This will help make a plan of intervention program for alcoholic patients.
