제 2형 당뇨병 환자의 당뇨관련 자기효능감과 자가관리 행위: 주관적 건강의 매개효과

국문 초록

본 연구는 제 2형 당뇨병 환자의 자가관리 행위에 영향을 미치는 요인을 확인하고 당뇨관련 자기효능감과 자가관리 행위에서 주관적 건강의 매개효과를 확인하는 것을 목적으로 실시하였다. J시의 일 병원에서 2015년 11월에서 12월 사이에 내분비 내과 외래를 방문한 연구 대상자 89명을 대상으로 구조적 설문지로 자료를 수집하였다. 자료는 IBM SPSS Statistics 23.0으로 기술통계, t-검정, 일원분산분석, Pearson 상관계수(correlation coefficient), 다중회귀분석, Sobel 검정(test)을 이용하여 분석하였다. 연구결과, 연구대상자의 당뇨관련 자기효능감의 평균평점은 71.47±13.35점이었고, 자가관리 행위의 평균평점은 3.53±1.18점, 주관적 건강의 평균평점은 2.89±0.91점이었다. 제 2형 당뇨병 환자의 당뇨관련 자기효능감은 자가관리(r=.359, p=.001)와 주관적 건강(r=.338, p=.001)과 양의 상관관계를 보였다. 주관적 건강은 당뇨관련 자기효능감과 자가관리 행위의 관계에서 부분적 매개효과를 나타내었다(β=.228, p=.034, Sobel test: Z=2.013, p=.044). 결론적으로, 본 연구 결과를 근거로 제 2형 당뇨환자의 자가관리 행위를 촉진하기 위하여 주관적 건강을 높이고 당뇨관련 자기효능감을 높일 수 있는 간호중재 전략을 구축할 필요가 있다.

영문 초록

The purpose of this study was to measure the mediating effect of subjective health status in the relationship of self efficacy and self care behavior of type 2 diabetes mellitus patients. The data were collected by questionnaires to objects who agreed with subject of this study. And 89 cases were analysis finally, t-test, one-way ANOVA, Pearson s correlation coefficients, multiple regression using Sobel test for mediation were conducted for data analysis using IBM SPSS 23.0. The result of this study, the mean score for self efficacy was 71.47±13.3, self care behavior was 3.53±1.18 and the mean for subjective health status was 2.89±.91. Subjective health status had a partial mediating effect (β=.228, p=.034) in the relationship between self efficacy and self care behavior (Sobel test: Z=2.013, p=.044). In conclusion, self efficacy was found to be associated with self care behavior in type 2 diabetes. Based on the findings of this study, nursing intervention programs focusing on increasing self efficacy, and increasing subjective health status are highly recommended to improve self care behavior in patients with type 2 diabetes.
