Equity of Medical Care Utilization in Korea: Using Concentration and HIwv Index

국문 초록

이 논문은 2008년부터 2013년의 한국의료패널 (KHP) 데이터를 이용하여 우리나라의 의료이 용에 대한 형평성에 대하여 보고자 한다. 의료이용은 의료비용, 외래횟수, 입원일수의 변수를 이용하기로 하며 의료 형평성 지수인 집중지수 (Concentration index)와 HIwv index를 사용하여 형평성을 살펴보고자 한다. 전반적인 의료이용의 형평성에 대해 구한 후 낙후 지역과 발전된 지역으로 나누어 그 안의 각 각 의료이용의 형평성을 구하였다. 성별, 결혼여부, 교육수준, 나이, 장애여부, 현주소, 만성질환 여부로 의료필요성을 예측하여 집중지수와 HIwv index를 구하였다. 2008년부터 2013년 총 6년간의 의료형평성에 대한 추세를 볼 수 있는 이 논문은 다음과 같은 결과를 보인다. 나라 전체로 보았을 경우 저소득층이 고소득층 보다 의료이용을 많이 하는 것으로 나타났 다. 발전된 지역과 낙후 지역 각각 안에서 외래이용과 입원일수는 모두 저소득층이 더 많이 사용하는 것으로 나타났으나 발전된 지역에서는 고소득층이 의료비 지출에 있어서 더 많이 지불하는 것으로 나타났다. 의료필요성을 통제한 후 결과는 외래이용과 입원일수는 저소득 층에게 치우친 불평등을 보이며 의료비 지출에 있어서는 고소득층에 치우친 불평등을 보인 다. 결과적으로 저소득층은 의료이용을 많이 하지만 의료지출을 더 많이 하는 것은 고소득 층으로 고소득층의 의료이용은 상대적으로 비교하였을 때 질적으로 상급수준의 의료이용을 하는 것으로 보인다..

영문 초록

We examine equity in medical care utilization from 2008 to 2013 using Korean Health Panel (KHP) data. We measure medical care utilization by medical expenditure, outpatient visit, and inpatient days. Concentration index (CI) measures inequality in utilization across socio-economic status and HIwv index (Wagstaff and Van Doorslaer 2000, 1803-1862) measures inequality after controlling for medical needs represented by sex, marital status, education level, age, disability, big city, and chronic disease variables. There have been several attempts to measure the equity in medical care utilization in Korea. However, while most of them focused on a single year, this paper studies the trend of equity for 6 years (2008~2013). Moreover, we studied equity across areas. The results showed that the poor use more medical care compared to the rich in Korea. Across diverse developed areas, the outpatient visit and inpatient days are pro-poor but medical expenditure are pro-rich in developed areas. In developed regions, the poor use more in outpatient visit and inpatient days compared to the rich whilst the rich pay more in medical expenditure compared to the poor. When medical need is controlled for, while the poor use more, the rich spend more. In conclusion, while the poor use more outpatient and inpatient services, it is the rich who spend more money on it. It appears that the rich may use more expensive and qualitatively different medical services compared to the poor.
