편측성 난청 성인의 인공와우이식 전과 후의 경험: 현상학적 질적 연구

국문 초록

배경 및 목적: 본 연구는 인공와우이식 전과 후의 편측성 난청 성인의 인식 및 경험을 의사소통, 신체 및 심리적, 사회적 측면에서 탐구하고 그 경험의 의미를 심층적으로 알아보고자 하였다. 방법: 인공와우이식을 받은 편측성 난청 성인 5명을 대상으로 인공와우이식 전과 후의 경험에 관한 심층 면접을 실시하였으며, 면담 내용을 현상학적 질적 연구 방법으로 분석하였다. 결과: 인공와우이식 전과 후의 경험에 대한 질적 분석 결과, 이식 전의 삶은 편측성 난청으로 인한 의사소통의 어려움으로 인해 신체 및 정신 건강 저하, 사회 참여 제한되는 부정적 피드백 사이클을 보여주었다. 반면, 인공와우이식 후에는 편측성 난청이 완화되면서 신체 및 정신 건강이 향상되고, 사회참여가 확대되면서 부정적인 피드백 사이클이 완화되었다. 그리고 인공와우 소리 질의 한계 및 향상에 대한 욕구를 확인하였다. 논의및 결론: 본 연구를 통해 편측성 난청이 삶의 질에 부정적인 영향을 미친다는 점과 인공와우이식을 통해 청력이 회복되면서 삶의 질이 향상된다는 점을 확인하였다. 이러한 결과는 조기 난청 발견 및 치료의 중요성을 강조하며, 편측성 난청에 대한 인식 확대, 경제적 및 심리적 지원제도의 필요성을 시사한다. 더불어, 수술 전 인공와우 소리의 음질에 대한 현실적인 기대치 설정, 음질 향상을 위한 전문가의 노력 및 지속적인 상담이 요구된다.

영문 초록

Objectives: This study aims to explore the experiences and perceptions of adults with single-sided deafness (SSD) before and after cochlear implantation, with a focus on the impacts on communication, physical and mental health, and social participation. Methods: A qualitative approach was employed, involving in-depth interviews with five adults who received a cochlear implant for SSD. The participants were selected through purposive sampling, and data were analyzed through Colaizzi’s phenomenological method. Results: The findings revealed that before cochlear implantation, participants experienced significant communication difficulties, a decline in physical and mental health, and social participation limitations. These challenges included difficulties in understanding speech in noisy environments, physical imbalance, anxiety, and reduced social interactions. Post-implantation, participants reported improvements in communication, as well as physical and mental health, and social participation. However, some participants noted limitations in the sound quality provided by the cochlear implant. Conclusion: Cochlear implantation substantially enhances the quality of life for adults with SSD by improving communication abilities, physical and mental well-being, and social engagement. Nonetheless, the study also highlights the need for ongoing support to address the limitations in sound quality and the associated psychological impacts.
