국문 초록

배경 및 목적: 본 연구의 목적은 이야기 담화 과제를 이용하여 파틴슨병((Parkinson’s disease, PD)과 정상압수두증(Normal Pressure Hydrocephalus, NPH) 환자의 인지-언어기능 저하를 민감하게 파악할 수 있는지 살펴보고, 더 나아가 두 집단 간 이야기 정보인출 수행양상에 차이가 있는지, 있다면 그 결과에 기반하여 두 집단의 감별진단이 가능한지에 대해 알아보는 것이다. 방법: 정상 노인(the healthy elderly, HE), PD 환자, NPH 환자각 20명씩을대상으로이야기정보인출과제를실시하여, 전체정보인출과부분정보인출의빈도를 비교하고, 정보인출 총점 및 정보인출 유형에 따른 수행력을 살펴보았다. 또한 이야기 정보인출 과제의 타당성을 확인하기 위해 K-MMSE-2와의 상관관계를 분석하였다. 결과: PD와 NPH 집단의 이야기 정보인출 수행력은 빈도로 측정하든, 점수로 평가하든 HE 집단보다 유의하게 떨어졌다. 그러나 정보 유형을 구분하여 비교한 경우에는 HE로부터 NPH 집단만을 선별할 수 있었다. 이야기 정보인출 과제 총점은 K-MMSE-2 점수와 유의한 정적 상관을 보였다. 논의 및 결론: 이야기 정보인출 과제를 통해 PD와 NPH 환자의 인지-언어능력이 정상군보다 유의하게 저하되어 있으며, 정보인출 유형에 따라 두 질환간 수행 양상이 다르게 나타날 수 있음을 확인하였다.

영문 초록

Objectives: The purpose of this study was to determine whether a story information retrieval task can be used to sensitively detect cognitive and language decline in patients with Parkinson’s disease (PD) and normal pressure hydrocephalus (NPH) and to differentially diagnose the two groups. Methods: To assess participants’ real-life cognitive and language abilities, we used the story information retrieval task, which involves showing a video of a story and then asking for information about it. The task was administered to 60 participants, 20 each of the healthy elderly (HE), PD patients, and NPH patients. We then compared the performance of story information retrieval among the three groups in terms of full and partial information frequencies, the total score, and scores by information type. And the correlation with the Korean version of the mini-mental state examination, 2nd edition (K-MMSE-2) was analyzed to check the validity of the story information retrieval task. Results: The PD and NPH groups performed significantly worse than the HE group on the story information retrieval, whether measured by frequency or total score. However, when compared by information type, only the NPH and HE groups showed significant differences. Conclusion: By assessing story information retrieval performance, we were able to identify language and cognitive decline in PD and NPH patients. And the study also showed that the two patient populations may perform differently when evaluated by information type.
