국문 초록

본 연구는 생태학적 체계이론을 기반으로 이동능력의 독립성에 따른 노인 우울의 영향요인을 분석하는 것을 목적으로 한다. 이를 위해 생태학적 요인을 개인요인과 지역사회요인으로 구성하고, 노인의 이동능력을 독립성에 따라 완전독립 이동능력, 부분도움 이동능력, 완전도움 이동능력 집단으로 구분한 후, 우울의 영향요인이 각기 다른지 검증하였다. 2020년도 노인실태조사 데이터를 사용하여 65세 이상 노인 9,885명을 연구대상으로 하였으며, SPSS Statistics를 사용하여 위계적 회귀분석을 시행하였다. 연구결과 완전독립 이동능력 집단에서는 인지기능, 도구적 일상생활수행능력, 주관적 건강 만족도, 삶의 만족도 등 개인요인과 생활편의시설 접근성, 사회적 활동 만족도, 지역사회환경 만족도 등 지역사회요인 모두 통계적으로 유의한 영향요인으로 검증되었다. 부분도움 이동능력 집단과 완전도움 이동능력 집단은 다소 상반된 양상을 보였는데, 부분도움 이동능력 집단에서는 인지기능, 주관적 건강 만족도, 삶의 만족도 등 개인요인이 주로 우울에 영향을 미친 반면, 완전도움 이동능력 집단은 지역, 사회적 존중, 생활편의시설 접근성 등 지역사회요인이 주요한 영향요인으로 검증되었다. 이와 같은 결과는 개인의 이동능력 독립성에 따라 우울에 영향을 미치는 요인이 각기 다르며, 이들의 우울을 예방하기 위해 집단 별로 특화된 접근이 필요함을 시사한다. 또한 노인의 이동능력에 따라 우울을 비롯하여 인지기능, 일상생활수행 기능, 삶의 만족도, 사회적 활동 만족도 등 다양한 심리사회적 요인에서 차이가 나타나고 있었다. 노인의 이동능력은 단지 개인의 신체적 자립성만의 문제가 아니라 사회정책적 기반에 따라 다양한 방식으로 보조될 수 있으며, 이는 노인의 이동권과도 관련되어 있다고 볼 수 있다. 따라서 노인의 이동권을 보호할 수 있는 사회제도적 장치가 마련되어야 할 것으로 생각된다. 이에 본 논문에서는 이동능력의 독립성 정도에 따라 노인 우울을 예방하고 감소시킬 수 있는 특화된 방법을 제안하고, 노인의 이동권을 보호할 수 있는 정책적 실현에 대한 계기를 마련하고자 한다.

영문 초록

This study aimed to analyze the factors influencing depression in older adults according to their mobility levels, and the ecological systems theory was used as a basis for the investigation. Ecological factors were composed of individual and community factors, and older adults’ mobility levels were divided into independent mobility, partial assistance mobility, and complete assistance mobility. This study analyzed data collected in 2020 from The National Survey of Older Koreans. Among the 10,097 older adults, 9,885 older adults aged 65 years and older were included in this study. We used a hierarchical regression model to analyze factors influencing depression in each group. In the group with independent mobility, such individual factors as cognitive function, instrumental activity of daily living, subjective health satisfaction, and life satisfaction, as well as community factors (e.g., accessibility to convenient living facilities, satisfaction with social activities, and satisfaction with the community environment) were all statistically significant. The partial assistance and complete assistance mobility groups showed somewhat opposite patterns. For those needing partial assistance, individual factors, such as cognitive function, subjective health satisfaction, and life satisfaction, mainly affected depression, while for those individuals requiring full assistance, community factors, such as region, social respect, and accessibility to convenient living facilities, were identified as major influencing factors. These findings suggest that the factors affecting depression differ depending on individuals’ mobility levels. Therefore, a specialized approach is needed for each group to prevent depression. Accordingly, we proposed a specialized method to prevent and reduce depression in older adults according to their levels of mobility.
