내시경-결합 신장내 결석수술 경험을 통한 의료기관 평가 표준화 방안에 관한 제안

국문 초록

신장 혹은 신우-요관 연결부의 거대 요로 결석은 대개 체외충격파쇄석술이나 역행성요관경수술로는 제거하기 어려워 경피적 신절석술 혹은 내시경-결합 신장내 결석수술이 필요하다. 이 난이도 높은 수술을 수행할 의료기관을 선택하기 위해서는 해당 병원이 요로 결석을 치료하기 위한 적정수준의 X-ray, 초음파 혹은 CT 장비 등을 보유하고 있는지와 의료진이 수술을 수행할 수 있는 충분한 경험을 갖추고 있는지를 고려하여야 한다. 본 논문에서는 신우-요관 연결부의 결석 제거 수술을 위하여 3개의 급이 다른 의료기관 즉 대학병원, 종합병원, 개인의원을 모두 방문하여 상담한 후 수술병원을 선택한 과정을 보고한다. 대학병원은 수술에 필요한 의료기기들을 잘 갖추고 있고, 수술 경험도 많지만 환자에게 검사와 수술 과정에 대하여 충분히 설명하지 않았고, 수술 일정을 정하는 것도 환자의 입장은 고려하지 않았다. 반면, 수술시설을 잘 갖춘 개인 의원 에서는 환자의 통증에 대하여 우선적으로 고려하고 검사 및 수술 과정도 상세히 설명하였으며, 수술 일정도 환자와 상의하여 결정하였다. 난이도 높은 수술을 받을 의료기관을 선정함에 있어서 의료기관의 시설 및 수술경험뿐 아니라 의료진의 환자와의 의사소통 및 태도가 중요한 요인이었다. 이 경험을 통하여 의료 전문가가 환자의 입장에서 참여하는 의료기관 평가 방안을 제안하며, 또한 시설과 인력을 충분히 갖춘 개인의원도 전문병원 지정에 포함시킬 것을 권한다.

영문 초록

Giant urinary stones at the renal pelvis-ureteral junction (PUJ) are usually difficult to remove with retrograde intrarenal surgery due to deformation of the ureter, requiring endoscopic-combined intrarenal stone surgery. In order to choose a medical institution to perform this highly difficult surgery, the patient should consider whether the hospital has an appropriate level of X-ray and CT equipments to treat this PUJ stone and whether the medical staff has sufficient experience to perform the surgery. In this paper, we report on the process of choosing a surgical hospital after visiting and consulting at three different level of medical institutions (a university hospital, a general hospital, and a private clinic) for PUJ stone removal surgery. The university hospital was well equipped with the medical devices needed for surgery and had enough experience of the surgery, but it did not sufficiently explain the test and surgery procedures to the patient, and did not take the patient's convenience into consideration at all when deciding on the surgery schedule. On the other hand, in a clinic with well-equipped surgical facilities, the patient's pain was given priority, the examination and surgical procedures were explained in detail, and the surgical schedule was also decided in consultation with the patient. When choosing a medical institution to undergo a difficult surgery, not only the facilities and surgical experience of the medical institution, but also the communication and attitude of the medical staff with the patient were important factors. Through this experience, we propose a method of evaluating medical institutions in which medical experts participate from the patient's perspective, and also recommend that private clinics with sufficient facilities and personnel be included in the designation of specialized hospitals.
