종합병원 일반병동의 간호업무량 상대가치 도출

국문 초록

본 연구에서는 자원기준 상대가치 산출모형을 적용하여 종합병원 일반병동의 간호서비스 생산에 소비되는 자원 추정을 위해 병원간호사회에서 제시한 48개의 기준간호행위에 대하여 서울과 경기도에 위치한 종합병원과 상급종합병원의 병동근무 간호사 385명에게 정맥내 점적주사 행위를 기준행위로 설정하여 간호기술 및 육체적 노력, 정신적 노력, 스트레스 등의 주관적 간호업무량을 상대적 강도 추정법(magnitude estimation)에 의해 조사하였다. 조사된 간호업무량 상대점수에 선행연구에서 제시된 표준간호시간을 반영하여 45개의 간호행위에 대한 상대가치를 도출하였다. 대상자의 경력과 상대가치의 관계는 대부분 역 상관관계를 보였으나 통계적으로 유의한 것은 채혈 등 10개 행위였다. 근무 부서별 상대가치 비교결과 체중측정 등 7개의 행위에서 통계적으로 유의한 차이를 보였고, 병원종별 상대가치의 평균은 상급종합병원이 종합병원에 비해 체중측정 등 20개 항목에서 통계적으로 유의하게 높게 나타났다.

영문 초록

The purpose of this study is to analyze and develop the relative value scales of nursing services provided in general wards of general hospitals. The magnitude estimation method was used to measure the relative value scales that were calculated by the weighted average of nursing skills, physical and psychological efforts, and mental stress. Intravenous drip injection was assumed as the baseline service of the 385 nurses who have been working in the wards of 2 general hospitals and 4 tertiary general hospitals located in Seoul and Gyeonggi province. The 48 standard nursing activities defined by the Hospital Nurses Association were compared to the baseline service and was able to deduce 45 standard nursing activities. The relationship between relative value and work experience of subjects mostly showed an inverse correlation. Nine activities including blood collecting procedures were statistically significant in a negative correlation. The relative values of seven activities including body weight measurement were statistically different among hospital departments. Between the two classes of hospitals, tertiary hospital obtained higher relative value scales in 20 nursing activities including body weight measurement.
