혈액투석 환자의 변비 완화를 위한 이압요법의 적용 효과

국문 초록

본 연구는 변비가 있는 혈액투석 환자에게 이압요법을 실시하여 변비 완화에 미치는 효과를 파악하기 위한 비동등성 대조군 전후 설계를 이용한 유사 실험연구이다. 연구 대상은 S시 소재 2개와 G시 소재의 1개, 총 3개의 인공신장실에서 혈액 투석 중인 변비 환자로 실험군 18명, 대조군 18명, 총 36명이었다. 실험군에게는 변비와 관련된 귀의 6개 자극점(대장, 직장, 간, 비장, 폐, 삼초)에 겨자씨를 붙인 후 5일 동안 1회 2초씩 적어도 하루 5회 이상 부착 부위에 압력이 느껴질 정도로 눌러주도록 하였다. 연구자는 5일 간격으로 총 3회의 이압요법을 적용하였다. 수집된 자료는 SPSS 19.0 프로그램을 이용하여 정규성 검정 후 모수검정법을 선택하였고 이압요법 적용 전 실험군과 대조군의 동질성을 검증하기 위해서 카이제곱검정(chi-square test), Fisher 정확검정(exact test) 및 독립 t-검정(independent t-test)을 시행하였다. 이압요법의 효과를 검증하기 위해 운동 횟수를 공변수로 처리하여 공변량 분산분석(analysis of covariance: ANCOVA)을 실시하였다. 연구 결과 이압요법을 적용받은 실험군은 적용받지 않은 대조군에 비해 변비사정 점수 변화에는 효과적이었으나(F=4.643, p=.039) 주당 배변 횟수 변화에는 차이가 없었다(F=0.183, p=.677). 따라서 이압요법을 위한 첩압 기간과 방법에 대한 후속 연구가 필요하여 지속적인 연구로 변비완화를 위한 독자적인 간호중재 방안으로 활용되기를 기대한다.

영문 초록

This study was to examination the effects of acupressure on constipation for hemodialysis patients. A quasi-experimental design with pretest and post-test measures was used. The subject of this study consisted of 36 patients (experimental group 18 and the control group 18), from 3 different hemodialysis clinics in S and G city. For the experimental group, small seeds (semen vacariae) were applied to six auricular acupoints for large intestine, rectum, liver, spleen, lung, and san jaio, because of their positive effects in relieving the symptoms of constipation. During interventions, participants were pressed to six auricular acupoints more than two seconds at least five times per day for 5 days. The seeds were applied once a 5 days for 3 times. Data were analyzed using the SPSS 19.0 program to perform chi-square test, Fisher s exact test, and independent t-test. Analysis of covariance was used to treat the covariate of the weekly frequency exercise time between experimental and control groups. There were statistically significant differences in the constipation assessment score in the experimental group (F=4.643, p=.039) but not in defecation frequency (F=0.183, p=.677). The results of this study indicate that acupressure could have an effect constipation for hemodialysis patients. As acupressure is a safe and effective nursing intervention, we recommend to apply it for hemodialysis patients.
