국문 초록

간병수발이 필요한 고령자가 인구고령화의 심화에 따라 늘어나고 있으며, 이들을 가족적 차원에서 보호해줄 수 있는 환경은 열악해지고 있어 간병수발의 문제가 사회적 리스크(social risk)화 되어가고 있으므로 정부는 장기요양보호정책을 다양한 각도로 준비해서 2008년 7월부터 시행하고 있다. 요양인정점수의 산출 및 등급판정 도구는 노인장기요양보험제도 시행에 있어 매우 중요한 요소이며 현재 사용하고 있는 요양인정점수 산출 도구는 데이터마이닝의 방법론인 의사결정분석(decision tree)의 결과를 이용하여 개발된 것이다. 본 연구에서는, 등급판정모형으로 일반화 가법모형(GAM : generalized additive models)의 적용가능성을 살펴보았다. 분석자료는 등급판정모형개발을 위해 조사된 고령자의 서비스량(시간) 조사자료를 이용하였으며, 분석결과 일반화가법모형이 고령자의 기능상태에 따른 서비스량의 비선형성을 잘 설명하고 있어 등급판정도구로의 가능성을 확인하였다. 특히 기존 의사결정나무에서 역전현상으로 설명이 어렵던 요인들이 일반화 가법모형에서는 종속변수와의 비선형 관계로 설명되어, 자료의 해석 및 영향력의 파악에 도움이 되리라 판단된다.

영문 초록

Long-term care insurance is social insurance system that provides benefits to the elderly who have difficulty taking care of themselves for a period of at least 6 months. It have been started in July, 2008. The assessment model is a very important factor to deploy the insurance system. In this study, we focus on the assessment of person’s functional conditions using the data of measures from service time survey 2011 in the view of generalized additive models (GAM). As a results, we can find that the generalized additive model could be the alternatives of decision tree model as assessment models in long-term care insurance because GAMs may explain the non-linearity of data. Especially, decision tree model is hard to explain the abnormal situations that is severe patients have a shorter service time. However, GAMs can explain the situations as non-linear relationships between service time and body condition scores.
