노인의 건강관련 특성이 자살생각에 미치는 영향

국문 초록

본 연구는 노인의 건강관련 특성이 자살생각에 미치는 영향을 파악하기 위해 전국 규모로 실시된 국민건강영양조사(Korea National Health and Nutrition Examination Survey, KNHANES) 제5기 2차(2011)년도 자료를 이용한 서술적 상관관계연구이다. 국민건강영양조사는 질병관리본부 연구윤리심의위원회의 승인을 받아 수행된 것이며(승인번호 L2011-02CON-06-C), 연구대상은 192조사구 3,840가구 10,589명 대상 중, 65세 이상 노인 1,598명이며, 조사기간은 2011년 1월부터 12월까지이다. 통계방법은 자살생각에 유의한 차이가 있는 독립변수를 다중로지스틱회귀분석(multiple logistic regression analysis)을 실시하여 오즈비, 절단값, 예측효율성을 구하였다. Model I은 심리적 건강특성을 보정한 후 위계적 회귀분석한 결과 스트레스가 적은 사람보다 많은 사람이 4.50배(CI 3.10-6.53) 우울증상이 있는 사람이 없는 사람보다 2.67배(CI 1.78-4.00) 자살생각을 할 확률이 높았으며 72.8%의 예측효율성이 나타났다. Model II는 심리적 건강특성, 신체적 건강특성을 보정한 후 회귀분석한 결과 활동제한이 있는 경우가 없는 경우보다 1.73배(CI 1.12-2.69), 자기관리(ADL)에 제한이 없는 사람이 있는 사람보다 0.54배(0.32-0.92) 자살생각을 할 확률이 높은 것으로 나타났으며, 예측효율성은 70.7%였으나, 양성예측도가 72.0%로 Model I보다 Model II에서 8.2%높았다.

영문 초록

This is a descriptive correlational study to investigate the effect of health characteristics on suicidal ideation in the aged by using data of a nation-wide survey, 5th Korea National Health and Nutrition Examination Survey (KNHANES). The survey performed from Jan to Dec, 2011 was approved by Institutional Review Board of Korea Centers for Disease Control and Prevention (No. L2011-02CON-06-C) and it included 1,598 elderly people among its total subjects, 10,589 from 3,840 households in 192 enumeration districts. To obtain odds ratio and 95% confidence interval statistically, multiple logistic regression analysis was conducted with independent variables showing a significant difference in suicidal ideation. Model I revising psychological health characteristics, found that the elderly with a high level of stress and depressive symptoms had more suicidal ideation by 4.50 times (CI 3.10-6.53) and 2.67 times (CI 1.78-4.00), respectively and its classification accuracy was 72.8%. Model II revising both of psychological and physical health characteristics, revealed that the elderly with activity limitation and without limitation on activity of daily living had more suicidal ideation by 1.73 times (CI 1.12-2.69) and 0.54 times (CI 0.32-0.92), respectively. As the predictive value of Model II was 72.0% although its classification accuracy was 70.7%, Model II was considered to predict suicidal ideation of the elderly better than Model I.
