도시지역 취약계층 노인의 우울과 판별예측요인

국문 초록

본 연구는 도시지역 취약계층 노인의 우울유무를 판별하는 예측요인을 규명하고자 시도되었다. 연구대상자는 B시의 16개 보건소에서 맞춤형방문건강관리서비스를 받고 있는 65세 이상 취약계층 노인 557명이며 수집된 자료는 SPSS/PC WIN 12.0 프로그램을 이용하여 기술통계 및 판별분석을 이용하여 분석하였다. 우울정도에 따라 정상군(n=284), 경증우울군(n=206), 중증우울군(n=67)의 세 집단으로 분류하여 우울을 판별한 결과 정상군과 중증우울군을 유의하게 판별하는 1개의 유의한 함수식이 도출되었으며 노인 우울변량의 36%를 설명하며, 정상군과 중증우울군을 79.5% 정확하게 판별하였다. 취약계층 노인의 우울유무를 유의하게 판별하는 예측요인으로 생활만족도가 가장 높게 나타났으며(-0.68), 다음은 허약수준(0.57), 인지기능(0.48), 주관적 건강인식(-0.44), 자기효능감(-0.41)이었다. 이상의 연구결과에 근거하여 도시지역 취약계층 노인의 우울을 감소시키기 위한 중재전략은 생활만족도, 허약수준, 인지기능, 주관적 건강인식, 자기효능감에 중점을 두어 개발하는 것이 효과적임을 알 수 있었다.

영문 초록

The purpose of this study was to examine discriminating factors of vulnerable elderly’ depression among life satisfaction, self efficacy, cognitive impairment level, perceived health status, frail level and number of chronic disease. The subjects were 557 vulnerable elderly aged over 65 who were receiving home care from one of 16 public health centers in B city. The data were collected using questionnaires and analyzed with descriptive statistics and discriminant analysis using SPSS WIN 12.0 program. The participants were grouped into three groups depending on the levels of the depression ‘normal group’ (n=284), ‘moderate depressed group’ (n=206), and ’severe depressed group’ (n=67). One function significantly discriminated between severe depressed group and normal group and classified 79.5% of original grouped cases correctly into the two groups. Life satisfaction was the most important factor having the highest discriminant loading as -0.68. And frail level(0.57), cognitive impairment level(0.48), perceived health status(-0.44) and self efficacy(-0.41) were significant discriminant factors of depression. Based on the findings of this study, developing intervention prgorams focusing on life satisfaction, frail level, cognitive impairment level, perceived health status, and self efficacy to reduce the depression of vulnerable elderly is suggested.
