국문 초록

본 연구는 지역사회 폐쇄성 환기장애 대상자들의 사회인구학적 특성, 건강행태 및 건강상태, 건강관련 삶의 질을 확인하고 건강관련 삶의 질과 관련된 요인을 분석하기 위한 것으로, 질병관리본부에서 시행한 국민건강영양조사 제6기 2차년도 자료의 일부를 이용한 이차분석연구이다. 연구대상은 폐기능 검사에서 ‘폐쇄성 환기장애’로 판명받은 성인이며 자료는 SPSS 23.0 프로그램을 이용하여 복합표본분석방법을 적용하여 분석하였다. 지역사회 폐쇄성 환기장애 대상자들의 건강관련 삶의 질 평균은 0.93± .001점이었으며 대상자의 16% 임상적 우울을 경험하였다. 폐쇄성 환기장애를 가진 본 연구대상자들의 각 특성별 준거집단이 비교대상이 되어 다른 집단과의 건강관련 삶의 질의 차이를 확인한 결과, 건강관련 삶의 질에 영향을 미치는 요인은 주관적 건강상태에서 매우 좋음(β=.114, p=.009), 좋음(β=.120, p=.003), 보통(β=.122, p=.003), 나쁨(β=.109, p=.006), 활동제한 있는 경우(β=-.173, p<.001), 우울수준(PHQ-9) 5점 이하(β=.054, p=.002)인 것으로 나타났다. 모형식은 F 통계량이 6.892(p<.001)로 유의하였으며, 모형의 설명력인 결정계수 .496이었다. 본 연구를 통해 지역사회 환기장애 대상자들의 건강관련 삶의 질 개선을 위해서는 활동장애와 우울 관련 요인을 규명하는 것이 선행되어야 함을 알 수 있었다.

영문 초록

The study aimed to investigate the sociodemographic characteristics, health behaviors, health status, health-related quality of life and to explore the predictors associated with health-related quality of life for obstructive ventilation disorder in community dwelling clients. The research design was a secondary analysis study, using data from the 6th Korea national health and nutrition examination survey (KNHANES VI-2), 2014, conducted by the Korea Centers for Disease Control and Prevention. The data were analyzed using complex sampling analysis methods by SPSS 23.0 program. The mean score of health-related quality of life was 0.93±0.001 and of the 16% obstructive ventilation disorder in community dwelling clients reported having a clinical depression. In general lineal model, influencing factors on health-related quality of life were ‘very good’ in subjective health status (β=.114, p=.009), ‘good’ (β=.120, p=.003), ‘moderate’ (β=.122, p=.003), ‘poor’ (β=.109, p=.006), ‘restriction of ambulation’ (β=-.173, p<.001), depressed level (PHQ-9) 5 points or less (β=.054, p=.002), which explained 49.6% of the variance (F=6.892, p<.001). To improve the health-related quality of life for obstructive ventilation disorder in community dwelling clients, it should be made a priority to identify specific factors that cause activity limitations and depression.
