국문 초록

한국과 일본은 고령자 돌봄 문제를 사회보장 방식으로 해결하기로 제도를 도입했으나, 보험으로 한 서비스 제공은 다른 결과를 초래하고 있다. 한국에서는 고령자 가구 형태가 독거노인, 고령부부 가구로 변화하고 있는 가운데, 일본에서 나타난 노노개호(老老介護) 문제는 일반적이지 않다. 본 논문은 이러한 현상에 대한 문제의식을 갖고 고령자 부부의 공동 생활을 지원하는 관점에서, 한일 돌봄 보험 제도의 구조적인 차이 및 고령 부부의 돌봄 현황을 밝혔다. 한국과 일본의 보험 제도는 요양이 필요로 한 고령자를 개인으로 간주함으로써, 제도를 통한 개인 지원과 고령자 부부가 필요로 하는 공동적인 지원 간의 모순이 존재하고 있다. 일본에서는 이러한 모순을 전제로 결국 고령 배우자가 돌봄을 맡는 반면, 이러한 모순은 수급자 및 공급자 간의 연결 및 조정을 담당하는 케어메니저가 직면하고 있다. 또한 한국에서는 수급자와 서비스 공급자 간의 연계 및 조정을 담당하는 제 3자인 케어매니저가 부재하기 때문에 개인사업소가 법적으로 허가된 배경에서 고령자 부부 간의 공동 생활이 공급자의 무상 행위로 일정 정도로 포용적으로 지원되고 있는 상황이 발생하고 있다.

영문 초록

Japan and South Korea have chosen to address the issue of elderly care through a social insurance approach;nevertheless, the provision of services through insurance has led to divergent outcomes. Despite decreasing family sizes, the phenomenon of simultaneous decline among elderly couples due to long-term care, as observed in Japan, is not as prevalent in South Korea. Acknowledging these realities, this paper illuminates the structural disparities within the long-term care insurance systems of Japan and South Korea, examining the practical aspects of elderly couple care through the lens of supporting their shared life. By considering the elderly in need of care as individuals, the insurance systems in Korea and Japan present a contradiction between the individual support emphasized by the system and the collective support required by elderly couples. In Japan, the ultimate responsibility for caregiving falls upon the elderly spouse, acknowledging these inherent contradictions. On the other hand, in Korea, where a third-party mechanism for coordination and collaboration between users and service providers has not been institutionalized, and with the allowance of individual providers, the shared life of elderly couples is to some extent inclusively supported by the altruistic actions of service providers.
