장애여부가 고혈압 및 당뇨병 이환에 미치는 영향

국문 초록

본 연구는 장애인과 비장애인간의 고혈압 및 당뇨병 유병에 미치는 요인을 파악하고 특히 장애보유 여부가 미치는 영향 살펴보고자 수행되었다. 이를 위해 2014년 장애인실태조사와 2012-2014년 국민건강영양조사 원자료 18,021건을 활용하여 장애인과 비장애인의 고혈압, 당뇨병 유병률을 산출하였으며, 가중치를 적용한 로지스틱 회귀분석 및 성향점수매칭법을 활용하여 장애가 고혈압 및 당뇨병 유병에 미치는 영향을 살펴보았다. 장애인의 유병률은 고혈압 44.67%, 당뇨병 22.38%로 비장애인에 비해 각각 1.79배, 2.43배 높았다. 로지스틱 회귀분석 결과 연령, 성별, 소득수준 등이 유의한 영향요인이었다. 특히 장애인은 비장애인에 비해 고혈압 유병의 가능성이 1.87배(95% 신뢰구간 1.70-2.06) 높았으며, 당뇨병 유병의 가능성은 1.78배(95% 신뢰구간 1.58-2.00) 높았다. 성향점수매칭 후 장애인과 비장애인간의 고혈압 유병의 차이(ATT)는 0.139였으며, 당뇨병의 경우 ATT는 0.082로 통계적으로 유의한 차이가 있었다. 연구결과를 통해 볼 때, 장애인에 대한 적극적인 만성질환 관리 정책이 필요하다.

영문 초록

The purpose of this study was to examine disparity in hypertension and diabetes prevalence among people with and without disability. Data of 18,021 people selected from the 2014 national survey for the people with disability and the 2012-2014 Korea national health and nutrition examination survey were used for weighted chi-square test, weighted logistic regression analysis, and propensity score matching. The prevalence of hypertension and diabetes in the disabled were 44.67% and 22.38%, respectively. Relative to the non-disabled, the odds ratio on the chronic disease prevalence were estimated 1.87 (95% CI 1.70-2.06) in hypertension and 1.78 (95% CI 1.58-2.00) in diabetes. After propensity score matching with other factors, the differences in disease prevalence between disabled and non-disabled people (ATT) were 0.139 in hypertension and 0.082 in diabetes. According to these results, it indicated that the prevalence of disability is a significant impact factor on chronic disease prevalence. This study suggested that management policies are needed to reduce health disparities of chronic disease for people disabilities.
