한국 성인의 흡연-비만 동시 발생과 우울의 관련성 : 2008년-2011년 국민건강영양조사를 바탕으로

국문 초록

본 연구는 한국 성인의 흡연-비만의 동시 발생과 우울의 관련성을 파악하기 위하여 국민건강영양조사 자료를 이용하여 이차 분석한 서술적 조사연구이다. 연구대상자는 국민건강영양조사 제4기와 제5기 중에서 체지방 검사를 포함한 2008년 하반기부터 2011년 상반기까지의 원시자료 중 만 20세 이상이면서 종속변수인 우울 여부에 결측 값이 없는 26,275명(남성 11,209명, 여성 15,066명)을 대상으로 하였다. 자료는 SPSS 20.0 프로그램의 복합표본 분석방법을 이용하여 Rao-Scott 카이제곱검정과 다중 로지스틱 회귀분석을 하였다. 연구결과 우울 유병률은 남성이 9.7%, 여성이 18.2%로 나타났다. 흡연-비만 동시 발생과 우울의 관련성을 보면 남성의 경우 비흡연-비비만 그룹에 비하여 흡연-비비만 그룹이 1.285배(95% CI=1.016-1.624) 더 우울한 것으로 나타났다. 여성의 경우는 비흡연-비비만 그룹에 비하여 흡연-비비만 그룹이 1.430배(95% CI=1.120-1.825), 우울-비만 그룹이 1.596배(95% CI=1.101-2.313) 더 우울한 것으로 나타났다. 그리고 남성의 우울에는 비만보다 흡연이 더 영향을 미치고, 여성의 우울에는 흡연보다 비만이 더 크게 영향을 미치는 것으로 나타났다. 따라서 흡연과 비만의 동시 발생이 우울에 미치는 영향을 고려하여 성별 특성에 부합되도록 우울 고위험군 선별 등의 체계적인 우울 예방 및 관리전략을 개발하는 것이 필요하다고 본다.

영문 초록

The purpose of this study was to investigate association of co-occurring smoking and obesity with depression among Korean adults using data from the Korea national health and nutrition examination survey (2008-2011). The total number of subjects in this analysis was 26,275 (men 11,209, women 15,066). Multiple logistic regression models were performed on the association co-occurring smoking and obesity with depression with complex sampling analysis by SPSS 20.0. The prevalence of depression was 9.7% in men, 18.2% in women. Among men, the OR for having depression was 1.285 (95% CI=1.016-1.624) in the smoking-nonobesity group, compared with the nonsmoking-nonobesity group. Among women, the ORs for having depression were 1.430 (95% CI=1.120-1.825) in the smoking-nonobesity group, 1.596 (95% CI=1.101-2.313) in the smoking-obesity group, compared with the nonsmoking-nonobesity group. In men, smoking has more relevance with depression than obesity, in women, obesity has more relevance with depression than smoking. It is necessary to adapt association co-occurring smoking and obesity with depression in screening of high risk group of depression for preventing and managing depression effectively.
