응급실 내원 환자의 한국형 중증도 분류 도구를 활용한 재분류 유형 분석

국문 초록

본 연구는 한국형 중증도 분류 도구(KTAS)로 분류된 환자 중 재분류가 이루어진 환자들의 특성을 분석하고, 환자의 특성에 따른 중증도 재분류 유형의 차이를 분석하고자 한다. 일개 지역응급의료센터에 내원하여 KTAS로 중증도가 분류된 환자를 전자의무기록(EMR)을 통해 추출한 연구대상은 총 531명이었고, 환자 요인과 분류자 요인으로 나누어 일반적 특성과 재분류 요인 및 재분류 형태를 SPSS version 21.0을 활용하여 기술통계, 교차분석(chi-square test), 일원분산분석(ANOVA) 등으로 분석하였다. 전체 531명의 재분류 유형은 비응급에서 응급으로 재분류되는 과소분류가 가장 많았으며, 중증도 변화 없이 동일하게 재분류되는 동일분류, 과대분류 순이었다. 환자의 상태변화 보다 분류자간 의견차이로 인한 재분류가 더 큰 비중을 차지했다. 분류자간 의견불일치로 인한 재분류의 경우 연령, 사고여부, 환자의 의식수준에 따라 재분류 유형에 유의한 차이를 보였다. KTAS는 우리나라의 표준화된 중증도 분류 도구로써 자리 매김하고 있으나 여전히 과소분류와 과대분류의 문제점을 안고 있음을 알 수 있다. 분류자간 일치를 위한 주기적인 교육가 요구되며, 재분류 가능성이 높은 대상자를 초기에 인식할 수 있도록 시스템을 구축하는 것이 필요하다.

영문 초록

To identify the potential problems of the patients classified as KTAS, the characteristics of the reclassified patients are identified, and the basic tools for improving the tools and classifiers are provided. The subjects of this study were 531 patients who were reclassified using KTAS among the patients admitted in the emergency rooms. The patients were divided into the factor of patient and the factor of classifier. Data was analyzed by descriptive statistics, chi-square test, ANOVA using SPSS (version 21.0). The re-triage due to discrepancy was 73.3%, which was more than the reclassification due to the patient condition change. In case of re-triage due to discrepancy, the re-triage pattern was a statistically significant difference in age, transportation, purpose, level of consciousness. Based on the results of this study, it can be seen that KTAS has become a standardized severity classification tool in Korea, but it still has problems of under triage and over triage. Therefore, there is a need to analyze the characteristics of the reclassified patients, develop educational programs through continuous research, improve and complement the tools, and provide basic data to solve the problems of KTAS.
