비디오 후두경을 사용한 기관내삽관 시 기존 탐침과 부지 탐침의 유용성 비교

국문 초록

본 연구는 외상으로 인해 목뼈 손상이 의심되는 경우 목 고정대를 사용하여 목을 고정한 후 신속하고 정확하게 기관내삽관을 하기 위한 방안을 모색하고자 한다. K광역시 H대학교 응급구조학과 재학 중인 4학년 중 전문기도관리술 과목을 이수한 학생으로 자발적 참여자 40명을 대상을 실시하였다. 실험 전 본 실험의 목적과 취지를 설명하고 서면으로 동의를 받고 실험을 진행하였다. 실험방법은 마네킹의 목을 목 고정대로 고정한 후 비디오 후두경으로 성문을 확인하고, 기존 탐침과 부지(Gum elastic bougie)를 이용한 삽관을 비교하였다. 연구 결과 기존 탐침보다 부지를 이용 경우 삽관 성공률에 통계적으로 유의한 차이가 나타났다(p< .044). 따라서 어려운 기관내삽관의 경우 기존 탐침보다 부지를 이용하는 비디오후두경 기관내삽관이 더 유용한 장비이다.

영문 초록

This study aims to find a way to quickly and accurately perform endotracheal intubation after fixing the neck using a neck fixture when it is suspected of damaging the neck bone due to trauma. Among the 4th graders enrolled in the Department of Emergency Rescue at H University in K Metropolitan City, 40 voluntary participants were surveyed as students who completed the professional technology course. Before the experiment, the purpose of this experiment was explained, and the experiment was conducted with written consent. In the experimental method, the neck of the mannequin was fixed with a neck fixture, and the glottis was confirmed with a video laryngoscope, and the intubation using the existing probe and a site was compared. As a result of the study, there was a statistically significant difference in the success rate of intubation when using a Gum elastic bagie than the existing probe (p<.044). Therefore, in the case of difficult endotracheal intubation, video laryngoscope endotracheal intubation using the site is a more useful device than the existing probe.
