예방적 의료서비스 이용에 미치는 영향 요인 분석

국문 초록

본 연구의 목적은 건강검진, 암검진 및 예방접종의 순응도를 높이고 질환의 예방 및 치료 비용을 절감할 수 있도록 기초 자료를 제공하는 것이다. 2020년 제8기 국민건강영양조사에 참여한 20세 이상 성인 남녀 중 무응답, 응답거부, 모름으로 답한 사람을 제외한 최종 5,340명을 분석하였다. 독립변수인 인구학적 특성, 사회경제적 요인 및 건강행태와 종속변수인 건강검진 수진, 암검진 및 인플루엔자 접종에 대한 복합표본교차분석 후, 종속변수에 대한 영향을 파악하기 위해 복합표본 로지스틱 회귀분석을 실시하였다. 인구학적 특성 및 사회경제적 요인에서 남성보다는 여성이, 나이가 증가할수록, 교육 수준이 높을수록, 배우자와 동거, 경제활동, 국민건강보험 가입, 민간의료보험 가입 및 소득수준이 높을수록 건강검진 및 암검진을 받는 경우가 증가하였다. 건강검진에서는 성별 유의적 차이가 없었고, 특히 인플루엔자 접종에서는 성별, 연령별, 혼인상태를 제외한 나머지 항목들은 유의적이지 않은 것으로 나타났다. 건강행태 요인에 따른 분석에서는 흡연 및 신체활동 정도가 건강검진, 암검진 및 인플루엔자 접종 여부에 영향을 주었지만, 통증/불편, 불안/우울, 음주 및 주관적 건강 상태 인식은 건강검진, 암검진 및 인플루엔자 접종 여부와 관련이 없는 것으로 확인되었다.

영문 초록

The purpose of this study is to provide basic data to increase compliance with health check-ups, cancer check-ups and vaccinations and to reduce the cost of preventing and treating diseases. Among the men and women aged 20 or older who participated in the 8th National Health and Nutrition Survey, the final 5,340 people were analyzed, excluding those who answered non-response, rejection, and unknown. After a complex sample cross-analysis on the independent variables such as demographic characteristics, socioeconomic factors, health behavior, and dependent variables such as health check-up examination, cancer check-up and influenza vaccination, a complex sample logistic regression analysis was conducted to determine the effect on the dependent variable. In terms of demographic characteristics and socioeconomic factors, women rather than men, as the age increased, the higher the level of education, and the higher the income level, the more they lived with their spouse, economic activity, national health insurance subscription, private medical insurance subscription, and receive health check-ups and cancer check-ups. There was no significant gender difference in health check-ups, and in particular, the remaining items except for gender, age, and marital status were not significant in influenza vaccination. In the analysis according to health behavior factors, smoking and physical activity affected health checkups, cancer checkups, and influenza vaccination, but pain/inconvenience, anxiety/depression, drinking, and subjective health status recognition were not related to health checkups, cancer checkups, and influenza vaccination.
