조음기관 훈련과 전기자극 치료의 병행 치료가 경직형 마비말장애 대상자의 호흡 및 조음 개선에 미치는 효과

국문 초록

이 연구는 다양한 장점을 갖추고 있지만 마비말장애 치료에서 체계적으로, 객관적으로 검증되지 않은 전기자극 치료와 기존의 조음기관 훈련을 동시에 경직형 마비말장애 대상자들에게 실시하여 호흡 및 조음 수행력의 개선 효과를 검증하고자 하였다. 뇌졸중이 발생한지 1년 미만이며, 우리말 조음·음운 평가(김영태·신문자, 2004) 결과 자음정확도가 60~70% 범위에 있으며, 구강 실행증이 동반되지 않은 33명을 최종 대상자로 선정하였다. 이 33명을 무작위로 세 집단으로 나누어, 각 집단마다 11명으로 맞추었다. 연구에서 전기자극 치료는 VitalstimⓇ(DJO Company, AP2116, USA)을 이용하였고, 조음기관 훈련 프로그램은 박정은(2009)의 연구에서 사용한 것을 이 연구 목적에 맞도록 수정·보완하여 사용하였다. 연구 결과는 다음과 같았다. 첫째, 세 가지 치료 실시 전·후 자음정확도에 있어 집단 간 유의한 차이가 있는 것으로 나타났다(p<.05). 유의한 차이가 있는 집단은 전기자극 치료 집단과 병행 치료 집단 간(p<.05), 조음기관 훈련 집단과 병행 치료 집단 간(p<.05)인 것으로 나타났다. 둘째, 세 가지 치료 실시 전·후 집단 간 최대발성시간에 있어 유의한 차이는 나타나지 않았다. 셋째, 세 가지 치료 실시 전·후 집단 간 조음기관 수행력에 있어 유의한 차이가 있는 것으로 나타났다(p<.001). 사후검증에서 유의한 차이가 있는 집단은 나타나지 않았지만, 평균을 고려해보면 병행 치료가 더 효과적이라고 말할 수 있다. 이러한 연구 결과들을 종합하여 고려해 보면 경직형 마비말장애 대상자들에게 조음기관 치료와 전기자극 치료를 각각 개별적으로 실시하는 것보다 병행 치료가 더 효율적으로 조음 수행력을 개선시키는 것으로 결론 내릴 수 있다.

영문 초록

This study was intended to demonstrate the effects of electrical stimulation therapy, which has a variety of known advantages but has not yet been proven systematically and objectively to be effective for dysarthria treatment, given in combination with the conventional articulatory training therapy on the improvements of breathing and articulation performance in the people with spastic dysarthria. The final subjects selected in this study were thirty-three (33) patients who experienced a stroke within less than a year, showed 60 to 70% of consonant accuracy based on the results of the Urimal Test of Articulation and Phonation (Kim Y. T. & Shin M. J., 2004), and did not have oral apraxia as comorbid disorder. These subjects were divided into three groups so that each group could be composed of eleven (11) subjects. In this study, VitalStim was used fro electrical stimulation therapy, while the articulatory training program devised by Park J. E.(2009) for her study was adapted and modified according to the objective of this study. The results of this study are summarized as follows. First, it was found that there were statistically significant differences in consonant accuracy between the groups before versus after the three different treatment options (p<.05). Specifically, such the differences were seen between electrical stimulation therapy group and combination therapy group (p<.05), and between articulatory training therapy group and combination therapy group (p<.05). Second, there was no statistically significant difference in maximum phonation time between the groups before versus after the three different treatment options. Third, there were statistically significant differences in articulatory performance between the groups before versus after the three different treatment options (p<.001). Finally, although any of the groups did not have a significant difference in post-hoc analyses, the calculated means seem to suggest that a combination of the two therapies is more effective. Based upon the above-mentioned results, it is concluded that a combination of articulatory training therapy and electrical stimulation therapy may lead to more efficient improvements in articulatory performance, compared to either alone, in the people with spastic dysarthria.
