국문 초록

유아사망률은 영아사망률과 더불어 그 나라의 사회경제적 수준 및 건강수준을 나타내는 지표 중 하나이다. 사회경제적 수준이 낮아질수록 유아사망률은 증가하며, 사회경제적 수준에 따른 유아사망 위험의 차이는 시간이 지날수록 지속되거나 증가하고 있다. 본 연구는 1995-2002년과 2005-2006년 출생 코호트(cohort)의 출생신고자료와 5년간 추적 관찰하여 얻은 사망신고자료를 연계한 자료를 이용하여 모의 연령과 출산순위에 따른 만 1세부터 만 5세 미만의 유아사망의 사회적 불평등을 기간별로 파악하고자 하였다. Cox 회귀모형을 이용하여 모의 연령 및 교육수준에 따른 유아사망 위험비를 구한 결과, 모 연령이 25세 미만이면서 모의 교육수준이 중학교 이하인 경우 유아사망률이 가장 높았다. 모 연령이 25세 미만인 경우에 사회적 불평등이 가장 심화됐으며 시간이 지나도 지속되고 있다. 출산순위가 첫째인 경우 1995-1996년, 1997-1999년에 사회적 불평등이 가장 심화되었으며, 2000-2002년에 출산순위가 둘째인 경우 사회적 불평등이 가장 심화되었다. 따라서 유아사망의 사회적 불평등은 나이가 어린 엄마와 출산의 경험이 적은 엄마에게서 가장 크게 나타났다.

영문 초록

Child mortality rate and infant mortality rate play roles of indicators of socioeconomic position of the country and national health level. Lower socioeconomic position yields higher child mortality rate. Difference in child mortality of socioeconomic position tends to continue or increase. This study investigated the social inequality in the mortality of child aged 1 to 4 according to maternal age and parity by using birth cohort in 1995-2002 and 2005-2006. Hazard ratio in child mortality was calculated according to maternal education level, age and parity using Cox regression model. The result of analysis showed that the group of children whose mother was aged less than 25 and educated middle school or lower had the highest child mortality rate. Social inequality in child mortality was greatest in the group of children whose mother was aged less than 25 and remained steady over time. Also social inequality in child mortality was the greatest for the parity one during 1995-1997 and for parity two during 1997-1999 and 2000-2002. Therefore, the children group whose mother was young or low parity showed the higher social inequality in child mortality.
