중․고령자의 우울에 영향을 미치는 요인 : 외로움, 웃음지수를 중심으로

국문 초록

본 연구는 중․고령자를 대상으로 우울, 외로움, 웃음지수 수준을 확인하고 일반적 특성에 따른 차이 및 우울에의 영향요인을 확인하는 것이다. 자료수집은 충청남도 도농복합도시인 C시에 거주하는 대상자로 일반적 특성을 포함한 우울, 외로움, 웃음지수 척도로 구성된 구조화된 설문지에 의하여 진행되었다. 수집된 자료는 SPSS/WIN 22.0을 사용하여 t-test, ANOVA, Pearson 상관계수(correlation coefficient), 위계적 회귀분석을 수행하였다. 연구결과, 우울 정도는 0～1점 사이에서 평균 0.37±.29점, 15문항의 총점 평균은 5.37±3.94점으로 나타났다. 총 점수 4점 이하인 정상인 경우가 65명(47.4%), 경증 우울 5～9점인 경우가 46명(33.6%), 10점 이상의 중증 우울이 26명(29.0%)이었다. 대상자의 외로움 정도는 점수범위 1～4점 사이에서 외로움은 1.99±.41점이었으며, 웃음지수는 점수 범위 1～5점 사이에서 3.54±0.58점으로 중간보다 약간 높았다. 위계적 회귀분석에 의한 우울에의 영향요인은 모델3에서 주관적 건강상태, 경제적 수준, 외로움이었다(F=9.228, p<.001). 따라서 노인뿐아니라 중장년층을 확대하여 만성질환을 가지고 있거나 재정적으로 취약한 대상자에 대하여 주기적으로 우울, 외로움, 웃음지수를 모니터링하고 중․고령층의 정서 심리적 간호를 위한 중재프로그램 개발을 제언한다.

영문 초록

The purpose of this study was to identify the emotional and psychological characteristics and the factors affecting depression in the elderly at home. Data collection was conducted by a structured questionnaire consisting of depression, loneliness, and laughter index including general characteristics. The data were analyzed using Pearson s correlation, t-test, ANOVA, and hierarchical regression analysis with SPSS/WIN 22.0. The degree of depression was 5.37 ± 3.94 point between 0 point and 15 points. 47.4% of the subjects with a total score of 4 points or less, 33.6% with mild depression from 5 to 9 points, and 29.0% with severe depression more than 10 points. The degree of loneliness of the subjects was 1.99 ± 0.41 between 1 and 4 points in the score range, and the laughter index was 3.54 ± 0.58 between 1 and 5 in the score range. Hierarchical regression analysis revealed that the factors influencing depression were subjective health status, economic level, and loneliness. Therefore, it is recommended to monitor intermittent depression, loneliness, and laughter index for vulnerable layers and develop intervention program for emotional and psychological nursing of the middle aged and elderly.
