퇴원손상심층조사 자료를 이용한 전립선비대증 입원환자 의료이용특성 분석

국문 초록

본 연구는 최근 10년간 보고된 퇴원손상심층조사자료를 이용하여 전립선비대증 환자의 의료이용현황과 재원일수를 파악하여 전립선비대증 환자의 간호문제 해결을 위한 기초자료를 제시하고자 시도하였다. 2008년부터 2017년까지 10년간 전립선비대증(N40)을 주진단으로 입원치료를 받은 총 7,036명의 입원환자를 대상으로 퇴원손상심층자료 분석을 위한 가중치를 적용하여 전국규모의 환자를 추정하였다. 연구결과 전립선비대증으로 입원한 환자는 총 203,583명으로 추정되었고 연령은 60세이상이 83.13%, 입원한 의료기관 특성으로 병원소재지는 서울, 경기, 인천(41.42%)이, 병상규모는 500-999병상(38.57%)이 가장 많았다. 입원경로는 외래입원(91.45%), 퇴원후향방은 귀가(99.18%), 수술종류는 경요도전립선절제술(58.29%)이 가장 많았고, 치료결과는 93.75%가 호전되었다. 총재원일수는 2일미만(34.28%)이, 수술후재원일수는 3-5일(46.22%)이 가장 많았으며, 총재원일수는 연령, 진료비지불방법, 병원소재지, 병상규모, 입원경로, 퇴원후향방, 수술종류, 부진단의 종류에 따라, 수술후재원일수는 연령, 병원소재지, 병상규모, 입원경로, 수술종류에 따라 통계적으로 유의한 차이가 있었으며 본 결과를 전립선비대증 환자간호 적절성 평가 및 간호계획을 위한 기초자료로 제시한다.

영문 초록

This research was conducted to provide a basis for solving nursing problems with BPH patients by analyzing data obtained from the Korean national hospital discharge in-depth injury survey of BPH patients from 2008 to 2017. This study shows that: a total of 203,583 patients were admitted to the hospital with BPH; 83.13% of the inpatients were over 60 years old; patients were mostly hospitalized in institutions in Seoul, Gyeonggi, and Incheon (41.42%); and the number of beds in these institutions was 500-999 (38.57%). This study further shows that most patients were admitted through outpatient care services (91.45%); most patients went home after discharge (99.18%); most patients received transurethral prostatectomy (58.29%); and patients conditions improved after surgery (93.75%). The length of stay (LOS) in hospitalization was less than 2 days (34.28%) and after surgery was 3-5 days (46.22%). There were statistically significant differences in the LOS according to age, payment method, hospital location, number of beds, hospitalization path, status after discharge, operation method, and the type of secondary diagnosis; and the LOS after surgery according to age, hospital location, number of beds, hospitalized path, and operation method. We propose that this study can be used as basic data for resolving nursing problems in patients with BPH.
