일반간호사의 근접오류 보고의향과 환자 안전 문화 인식간의 관계

국문 초록

본 연구는 일반간호사의 근접오류 보고의향과 환자 안전 문화 인식을 파악하고 주요변수들간의 관계를 파악함으로써 근접오류 보고에 대한 체계 도입 및 개선을 위한 기초자료를 제공하기 위해 시도되었다. 서울, 경기도, 강원도 소재 5개 상급종합병원에 근무하는 일반 간호사 472명을 대상으로 구조화된 설문지를 이용하여 대상자의 일반적 특성과 근접오류 및 보고에 대한 인식 수준, 경험 여부, 근접오류 보고의향, 환자 안전 문화 인식 수준을 측정하였다. 수집된 자료는 기술통계, 독립성 t-검증, 일원분산분석, 상관관계분석을 시행하였다. 연구결과 전반적인 근접오류 보고의향은 6.82점(10점 척도)이었으며, 10개의 근접오류 상황 중에서 낙상과 투약의 보고 의향 점수가 높았다. 환자 안전 문화 인식은 3.30점(5점 척도)이었다. 근접오류 보고의향은 연령, 학력, 근무경력, 안전교육 횟수, 근접오류 인지수준, 보고체계 인지수준, 근접오류 보고 경험과 유의한 차이를 보였으며, 환자 안전 문화 인식과는 양의 상관관계(r=.280. p<.001)를 나타냈다. 근접오류 보고를 촉진시키기 위해서는 환자 안전 및 근접오류 보고에 대한 지속적인 교육과 함께 조직문화의 변화, 인식 전환을 위한 체계적인 노력이 필요하다.

영문 초록

This study was conducted to provide basic data for introducing and improving the system of near miss reporting by grasping the relationship between general nurses’ willingness to report near miss and perception of patient safety culture. The participants were 472 nurses from four different university hospitals located in Seoul, Gyungi-do, and Kangwon-do. Data were analyzed with statistical program SPSS WIN 20.0 using descriptive statistics, independent sample t-test, one-way ANOVA, and Pearson s correlation. The findings showed that the score of willingness to report near miss was 6.82 (10-point scale) and patient safety culture perception was 3.30 (5 point scale). Of the 10 common near miss items, there was a high level of willingness to report near miss for falls and medications. The willingness to report near miss was positively correlated with the perception of patient safety culture and significantly different from the age, education, work experience, number of safety education, awareness of near miss, awareness of reporting system, and experience in near miss. There should be systematic efforts to shift awareness, along with continuing education on patient safety and near miss reporting.
