Area-level Deprivation and Mortality in Subjects with Lung Cancer: A Survival Analysis from Busan, Korea

Socioeconomic status has been implicated in cancer but little study has simultaneously analyzed the potential effect of area-level deprivation on lung cancer mortality in Korea. The purpose of the present paper is to analyze whether there is an association between area-level deprivation and mortality rates of lung cancer in Busan, Korea. The mortality cases of lung cancer were determined in the entire Busan population of all age between 2000 and 2011. Cox proportional hazard model was used in the analysis to study the effects of multiple risk factors such as gender, age, and the level of area-level deprivation in local area. An area-level deprivation index, put together from the education and income level, unemployment status, and welfare assistance, was used to assess the level of area-level deprivation. There was a strong association between level of area-level deprivation and mortality of lung cancer. Increasing area-level deprivation index was found to be significantly associated (p-value < 0.05) with higher mortality of lung cancer after adjusting for gender, age, and year of diagnosis. Compared to the least deprived group, the hazard ratios (95% confidence interval) were 1.08 (1.03, 1.13), 1.11 (1.05, 1.17), 1.08 (1.01, 1.15) for the second, the third, and the fourth (most deprived) quartile of area-level deprivation index groups, respectively. Results suggest that area-level deprivation is associated with survival rate mortality cases of lung cancer in Busan, Korea.
