전반적 자폐 표현형과 우울의 관계: 사회불안의 매개효과

국문 초록

전반적 자폐 표현형(broad autism phenotype, BAP)는 자폐 장애와 유사한 특징을 보이지만, 증상이 자폐 스펙트럼 장애의 진단기준을 충족시키지 못하는 경미한 형태로 나타나는 것을 의미한다. BAP는 보통 자폐 스펙트럼 장애 진단을 받은 환자의 가족과 친족에서 확인되지만, 일반인구에서도 BAP 특성이 높게 관찰될 수 있다. BAP 집단은 일반 인구에 비해 우울 수준이 높은 것으로 나타나는데, 그 기제에 대한 고려는 부족한 실정이다. 이에 본 연구에서는 BAP가 우울에 영향을 미치는 기제로서 사회불안의 역할을 검증하고자 하였다. 이를 위해 대학생 495명을 대상으로 전반적 자폐 표현형 척도(BAPQ), 사회적 상호작용 불안 척도(SIAS), Beck 우울 척도-II (BDI-II)를 실시하였고, 수집된 자료에 대해 기술통계분석, 상관분석 및 매개효과 검증을 실시하였다. 분석 결과, 세 변인 간 정적 상관관계가 확인되었다. 그리고 전반적 자폐 표현형과 우울 간의 관계를 사회불안 정도가 부분매개하는 것으로 나타났다. 이러한 결과는 전반적 자폐 표현형을 보이는 개인의 우울 증상을 개선하고자 할 때 사회불안과 같은 대인관계 관련 특징을 고려하는 것이 필요함을 시사한다.

영문 초록

A broad autism phenotype (BAP) is a milder expression and subclinical level of autistic traits. BAP is typically observed in the families and relatives of individuals diagnosed with ASD, although such traits can also be found in the general population. The BAP group is known to show a higher level of depression than the general population, but the mechanism has not been well explored. This study aimed to examine the role of social anxiety in the relationship between the broad autism phenotype and depression. To this end, we administered self-reports assessing the level of broad autism phenotype, social anxiety, and depression to 495 university students, and conducted correlation and mediation analyses on the collected data. Our results showed that broad autism phenotype, social anxiety, and depression were positively correlated. In addition, it was found that social anxiety partially mediates the relationship between broad autism phenotype and depression. These findings suggest that it is necessary to consider interpersonal features such as social anxiety when treating depressive symptoms in individuals with a broad autism phenotype.
