병원 입원 경험이 있는 노년층의 환자 만족도 관련 요인

국문 초록

본 연구의 목적은 병원 입원 경험이 있는 노년층이 입원하는 동안 느낀 전반적인 만족도와 관련된 요인을 확인하는 것이다. 이를 위하여 2019년 보건복지부 의료서비스 경험 조사에 참여한 12,507명의 자료 중 병원 입원 경험이 있는 만 60세 이상 성인 294명의 자료를 분석하였다. 자료분석을 위하여 빈도와 백분율, 평균과 표준편차, 독립표본 T 검증, Pearson 상관관계, 가중치를 적용한 다중 회귀분석을 이용하였다. 그 결과, 대상자의 만족도를 증가시키는 관련 요인은 기대했던 치료 결과 달성(β=0.39, p<.001), 긍정적인 병원 환경(β=0.33, p<.001)과 의사의 태도(β=0.22, p=.006)였다. 반면, 간호사의 태도(β=-0.13, p=.072), 의료인의 환자 안전 활동에 대한 대상자의 인식은 만족도를 높이는 유의한 관련 요인이 아니었다. 그러나 환자 안전 활동 중 손 위생은 대상자의 26.2%가 ‘의료인이 비경구 투약 전 손 위생을 하지 않았다’라고 응답하였고, 단변량 분석에서 ‘의료인이 손 위생을 수행했다’라고 응답한 대상자의 만족도가 유의하게 높았다(t=-2.43, p=.017). 그러므로, 의료인은 손 위생에 더욱 주의를 기울일 필요가 있다. 본연구결과를 바탕으로, 의료기관은 주 고객인 노년층의 만족도를 높이기 위하여 개선해야 할 요건을 점검할 필요가 있다.

영문 초록

The purpose of this study was to identify factors related to patient satisfaction in hospitalized older adults. Out of 12,507 people who participated in the patient experience survey conducted by the Ministry of Health and Welfare in 2019, 294 adults aged 60 or older who were hospitalized, were analyzed. For data analysis, we utilized descriptive statistics, independent sample T test, Pearson correlation, and multiple regression analysis with weights. As a result, relevant factors increasing patients’ satisfaction were achievement of the expected outcome of treatment (β=0.39, p<.001), positive hospital environment (β =0.33, p<.001) and regular doctor’s attitude (β=0.22, p=.006). However, nurse s attitude and patient safety activities were not significant related factors that increased patient satisfaction in older adults. Additionally, 26.2% of participants in the single-variable analysis said that health care provider did not perform hand hygiene before giving medication. Hand hygiene of health care providers significantly increased patient satisfaction (t=-2.43, p=.017). Therefore, they need to pay more attention to hand hygiene. Based on the results of this study, it is necessary to check various requirements that hospitals should improve in order to increase the patient satisfaction of the elderly.
