범이론적 모형을 적용한 고혈압 노인의 약물순응행위에 관한 연구

국문 초록

본 연구는 고혈압 노인의 약물순응행위 변화단계를 파악하고, 약물순응행위 변화단계와 관련된 변화과정, 자기효능감, 의사결정 균형의 차이를 파악함으로써 고혈압 노인의 약물순응도 향상을 위한 효과적인 중재 프로그램을 개발하는데 필요한 기초 자료를 제공하고자 한다. 본 연구의 대상자는 65세 이상 노인 중 고혈압으로 진단받은 지 1년 이상 약물을 복용하고 있는 272명을 편의표본추출방법에 의해 선정하였고, 2010년 7월 1일부터 30일까지 구조화된 설문지를 이용하여 자료수집 하였다. 본 연구결과 약물순응행위 변화단계에 따라 인지적 변화과정의 의식향상과 행위적 변화과정의 자아해방, 자극조절, 강화관리, 역조건화가 유의한 차이를 보였다. 또한 약물순응행위 변화단계에 따라 자기효능감과 의사결정 균형 중 약물순응 이익에서 유의한 차이가 있는 것으로 나타났다. 따라서 고혈압 노인 약물순응도 향상을 위한 중재프로그램 개발 시 행위적 변화과정, 자기효능감과 약물순응 이익 등의 전략을 적극 활용하여야 할 것이다.

영문 초록

The purpose of this study was to investigate the stages of change, processes of change, decisional balance, and self-efficacy of medication adherence behavior among elderly with hypertension based on transtheoretical model. The study included 272 elderly with hypertension participants. The findings indicated a meaningful difference between the stages of change and experience of hypertension side effect. This study founded statistically significant differences between the stages of change and consciousness raising among the cognitive process of change. And the study founded statistically significant differences between the stages of change and self liberation, stimulus control and reinforcement management among the behavioral process of change. Statistically significant differences were also evident between the stages of change and self-efficacy and the benefits of medication adherence among the decisional balance. Based on the above findings, an intervention strategy was devised to improve medication adherence for elderly with hypertension.
