퇴원손상심층조사 자료를 이용한 최근 5년간의 방광암 환자 분석

국문 초록

본 연구는 퇴원손상심층조사 자료를 이용하여 최근 5년간 방광암 환자의 의료이용현황과 재원일수를 파악하여 방광암 환자의 간호문제해결을 위한 기초자료로 제시하고자 시도되었다. 2005년부터 2009년까지 5년간 방광암(C67.0-C67.9)을 주 진단으로 입원치료를 받은 2,163명을 최종 분석대상자로 선정한 후 퇴원손상심층자료 분석을 위한 가중치를 적용하여 전국규모의 환자를 추정하였다. 연구결과 방광암 환자는 남자가 여자보다 많았고 연령은 55세 이상이 85%였으며, 의료기관 특성으로 병원소재지는 서울, 경기, 인천이, 병상규모는 500-999병상이 가장 많았다. 의료이용 특성으로 입원경로는 외래를 경유한 경우, 퇴원 후 향방은 귀가가, 수술종류는 경요도방광절제술이 가장 많았고, 치료결과는 92.82%가 호전되었으며 인구 10,000명당 입원건수는 2005년 2.05명, 2006년 2.03명, 2007년 1.78명, 2008년 1.94명, 2009년 2.11명이었다. 평균재원일수는 8.439일이었으며 연령, 의료보장형태, 병상규모별, 입원 경로, 치료 결과, 퇴원 후 향방, 암 발생부위에 따라 재원일수의 차이가 있었으며 본 연구결과를 추후 방광암 환자의 간호문제해결을 위한 기초자료로 제안하고자 한다.

영문 초록

This research was carried out to provide a basis for solving nursing problems with bladder cancer patients by analyzing the data, obtained from the Korean national hospital discharge in-depth injury survey (KNHDS), on the status of medical services and the length of hospital stay of bladder cancer patients from 2005 to 2009. Total 2,163 hospitalized subjects, who had primarily diagnosed with bladder cancer (C67.0-C67.9), were selected for the analysis, and the data was applied with a weighting function to estimate the status of national patients. The bladder cancer patients in male are more than female, 85% of them are older than age 55, the hospitals the patients have stayed are mainly located in the areas of Seoul, Gyeonggi, and Incheon, due to better medical facilities, and the most common hospitals had 500-999 beds. As a characteristics of hospital utilization, more patients entered the hospital through outpatient care services, more patients went home after discharge, more operations were done in trans urethral resection of bladder, the 92.82% of the operation results were improved, and the numbers of hospitalization were 2.05 people in 2005, 2.03 people in 2006, 1.78 people in 2007, 1.94 people in 2008, and 2.11 people in 2009 per 10,000 population. The average length of stay was 8.439 days and there were some difference in the length of stay with age, health care condition, size of beds, hospital routes, treatment outcomes, attitude after discharge, and the place of cancer. In this study, the summary data of discharged bladder cancer patients is proposed to be a basis for solving the nursing problems with bladder cancer patients.
