자궁근종 여부에 영향을 미치는 DSOM 변수의 비교분석

국문 초록

자궁근종은 여성의 자궁 및 골반 내에서 발생하는 양성종양 중 발생빈도가 가장 높은 것으로 특히 30세 이후에 발생빈도가 높아지는 질환으로 알려져 있다. 최근에 동의대학교 한방부인과에서는 실제 자궁근종 환자가 가지고 있는 증상에 대한 분석을 수행한 바 있으며, 이를 좀 더 발전시켜 자궁근종 증상지표를 개발하는 연구를 시행하고 있다. 본 연구에서 고려하는 실험군은 2001년 5월부터 2006년 6월까지 동의대학교 한방부인과에 내원한 환자 중 자궁근종이 있는 환자 252명이고, 대조군은 서로 다른 세 집단이다. 첫 번째 대조군은 2005년 6월부터 2006년 12월까지 DSOM(한방진단시스템)으로 진단을 받았으나 자궁근종이 없는 외래환자 727명이고, 두 번째 대조군은 2005년 11월부터 2006년 1월 사이에 사상체질의학과와 한방부인과에서 시행한 임상시험에 참여한 사람으로서 부인과 질환이 있는 환자와 정상인이 무작위로 섞여 있는 집단 127명이고, 세 번째 대조군은 2005년 3월부터 2005년 4월까지 한방진단시스템의 문항을 추가하기 위하여 조사한 건강한 여자대학생 197명이다.본 논문에서는 단계별 로지스틱 회귀모형을 이용하여 세 가지 대조군에 따라 환자들의 자궁근종 여부에 영향을 미치는 DSOM 변수를 찾고, 진단정확률과 민감도, 특이도를 제시하고 있다.

영문 초록

Uterus myoma is a benign tumor of smooth muscle in the wall of the uterus. Recently, in Oriental Medicine, the concerns of uterus myoma patients has been increased. We analyze the medical records for 1051 outpatients and college women including 252 uterus myoma patients who visited Dongeui University Oriental Medical Center from May 2001 to June 2006. We investigate the DOSM symptom scores which effect uterus myoma patients using stepwise logistic regression model. Logistic regression analysis indicate as follows; In case of control group composed of 727 outpatients, 22 items of DSOM were associated with the myoma, 12 items are associated positively and 10 items negatively, and the results show that correct rate is equal to 80.8%, sensitivity is 72.6%, specificity is 83.6%.In 127 clinical trials data, 20 items of DSOM were associated with the myoma, 12 items are associated positively and 8 items negatively, and the results show that correct rate is equal to 85.2%, sensitivity is 83.3%, specificity is 89.0%.In 197 college women data, 23 items of DSOM were associated with the myoma, 14 items are associated positively and 9 items negatively, and the results show that correct rate is equal to 95.8%, sensitivity is 96.4%, specificity is 94.9%.In 1051 outpatients, clinical trials data and college women data, 28 items of DSOM were associated with the myoma, 14 items are associated positively and 14 items negatively, and the results show that correct rate is equal to 80.4%, sensitivity is 78.2%, specificity is 81.0%.
