수부손상 재접합 환자의 신체상, 외상 후 스트레스가 우울에 미치는 영향

국문 초록

본 연구는 수부손상 재접합 환자들의 신체상 및 외상 후 스트레스, 우울 수준을 확인하고 대상자의 우울에 영향을 미치는 요인을 확인하였다. 자료수집은 경기도 S시 소재 일 개 종합병원에서 수지 절단으로 접합 수술을 받은 후 30일이 경과한 환자를 대상으로 신체상, 우울, 외상 후 스트레스를 포함한 일반적 특성으로 구성된 구조화된 설문지에 의하여 진행되었다. 수집된 자료는 SPSS/WIN 21.0을 사용하여 t-test, ANOVA, Pearson 상관계수(correlation coefficient), 단계적 다중회귀분석(stepwise multiple regression analysis)으로 분석하였다. 본 연구결과, 대상자의 신체상 정도는 3.24±.41점(1~5점)으로 중간 정도였으며, 외상 후 스트레스 정도는 1.18±.61점(0~3점)으로 중간보다 약간 낮았다. 우울 정도는 1.06±.45점(0~3점), 평균 21.31±9.03점(0~60점)으로 나타났다. 총 점수 24점 이상인 우울집단이 37.0%(37명)에 달하였다. 대상자의 우울에 영향을 미치는 요인으로는 신체상(β=-.311), 외상 후 스트레스(β=.493), 일상생활 불편감 정도(β=.153) 순으로 나타났다. 회귀모형은 유의하였으며(F=38.553, p<.001), 설명력은 53.2%이었다. 따라서 수부손상 후 특히 일상생활의 불편을 경험하는 대상자에 대하여, 기능적인 재활에만 초점을 맞추는 것이 아니라, 재활과 함께 우울을 모니터링하여 정서 심리적 간호중재가 함께 이루어져야 할 것이다.

영문 초록

The purpose of this study was to identify the factors affecting depression in patients with hand injuries. The participants were 100 outpatients with hand microsurgery in C hospital in Gyeonggi_do. The data were analyzed using Pearson s correlation, t-test, ANOVA, and multiple regression with SPSS/WIN 22.0. The body image of the subjects was 3.24±0.41 points (1~5 points) and the post - traumatic stress level was 1.18±.61 points (0~3 points). The degree of depression was 1.06±0.45 points (0~3 points) and the mean was 21.31±9.03 points (0~60 points). And 37.0% of depressive patients with a total score of 24 points or more. The factors affecting the depression of the subjects were body (β=-.311), posttraumatic stress (β=.493), and daily life discomfort (β=.153). Regression model was significant (F=38.553, p<.001) and explanatory power was 53.2%. Therefore, for hand microsurgery patients those who experience discomfort during daily life, it is necessary not only to focus on functional rehabilitation but also to provide emotional and psychological nursing intervention with rehabilitation.
