국문 초록

목 적: 급성 위장관염에 동반된 양성 경련(benign convulsion associated with acute gastroenteritis, BCAGE)은 임상에서 흔히 경험할 수 있는 질환이지만 간질 및 간질증후군의 국제 분류에 기술되어 있지 않으며 최근 많은 연구가 진행되고 있으나 영상진단검사 및 치료에 대한 연구는 드물다. 본 연구에서는 BCAGE의 이들을 포함한 임상적 특성에 대하여 연구하고자 하였다. 방 법: 2000년 3월부터 2004년 10월까지 5년간 전남대학교병원 소아과에서 위장관 증상과 경련으로 치료받았던 환아 52명 중 BCAGE로 진단받은 환아 28명(남 17명, 여 11명)을 대상으로 각 환아의 연령별, 성별, 월별 분포, 경련의 발생 시기, 형태, 지속시간, 횟수, 뇌파검사, 영상진단검사 소견, 치료 등을 후향적으로 조사하였다. 결 과: BCAGE의 발병 연령은 1개월에서 96개월 사이였고, 10월에서 2월 사이에 가장 많이 발생하였고, 대부분 전신 발작으로 전신-강직성 발작이 19명(67.9%)으로 가장 많았다. 경련의 횟수는 1-7회(2.7±0.9)였고, 이 중 2-4회가 대부분 (75%)을 차지했다. 경련 지속시간은 대부분 1-5분이였고, 15분 이상 경련을 한 경우도 2례(7.1%)가 있었다. 위장관 증상이 시작된 후 2-4일에 대부분(85.7%)의 경련이 발생되었다. 대변의 Rotazyme 검사를 시행한 18명 중 3명(16.7%)에서만 양성반응을 보였으며, 대변 배양검사에서 균이 동정된 경우는 없었다. 뇌척수액 검사는 모두 정상 소견을 나타내었다. 뇌파검사를 시행한 24명 중 20명(83.3%)은 정상 소견을 보였으나, 4명(16.7%)은 비정상 소견을 보였다. 이중 1명에서만 추적검사가 시행되었고 정상화되었음을 확인하였다. 17명에서 CT 등의 뇌영상진단 검사를 시행하였고, 이 중 6명에서 경도의 뇌부종이 관찰되었으며 우측 전두엽과 두정엽의 피질이형성증 1례, 소뇌 후부 뇌척수액 저류 1례가 관찰되었다. 17명의 환아에서 응급실에 도착하여 경련 조절을 위해 diazepam을 사용하였고 이들 중 12명은 입원하여 phenobarbital과 phenytoin을 투여하였다. Diazepam을 사용하지 않았던 11명의 환아 중 2명이 입원하여 항경련제를 투여하였다. 4명의 환아는 퇴원 후 최장 12개월까지 항경련제를 복용하였다. 결 론 : BCAGE는 임상에서 흔히 경험할 수 있는 상황 연관성 양성 경련으로서 장기간의 항경련제 투여가 필요하지 않으며 예후가 좋은 질환이지만 원인 및 병태생리에 대한 연구가 더 필요할 것으로 사료되었다.

영문 초록

Purpose : The aim of this study was to reveal detailed clinical features of benign childhood convulsions associated with acute gastroenteritis(BCAGE). Methods : The medical records of 28 patients with BCAGE, who were admitted to the department of pediatrics, Chonnam National University Hospital between March 2000 and October 2004, were reviewed on the aspects of clinical symptoms, diagnostic tests and treatment. Results : The ages of the subjects cases ranged from one to 96 months old and most of them had BCAGE in late autumn and winter seasons. Seizures were developed mostly 2 to 4 days after the symptoms of gastroenteritis appeared. Most of them showed generalized seizures which occurred repetitively during the episode of acute gastroenteritis (mean 2.7±0.9 times of the seizures attack), mostly 2-4 times. The duration of seizure was largely below 5 minutes, but in 2 cases it was prolonged over 15 minutes. The Rotazyme tests of stool were positive only in 3 cases(16.7%), and the cerebrospinal fluid and blood examinations revealed normal results. Either brain CT or MRI was performed in 17 patients and variable results were demonstrated. Most of them showed normal or mild brain swelling, but one showed cortical dysplasia of the frontal and parietal lobes, while another one showed cerebrospinal fluid collection in the posterior fossa. 17 patients were treated with diazepam, 12 of whom were prescribed additional phenbarbital or phenytoin. 2 patients who did not take diazepam were administered antiepileptics. Antiepileptic drugs were continuously given to 4 patients for up to one year after discharged from hospital. During follow-up periods, there occurred no further seizures in all the patients. Concl usi on : BCAGE can be considered as a situation related seizure which does not need any long-term antiepileptic medications.
