어머니-영유아 상호작용에서 어머니의 단서 유형에 따른 인공와우이식 영유아의 공동주의 특성

국문 초록

배경 및 목적: 본 연구는 인공와우이식 집단과 건청 집단의 부모-영유아 상호작용에서 나타나는 공동주의 특성을 비교해보고, 인공와우이식 집단에서 공동주의 변수와 영유아의 언어 점수와의 관련성을 살펴보고자 하였다. 방법: 인공와우이식 영유아와 어머니 13쌍, 생활연령을 일치시킨 건청 영유아와 어머니 17쌍이 연구에 참여하였다. 대상자들은 자유로운 상호작용 과제에 20분 동안 참여하였다. 결과: 어머니-영유아 상호작용에서 인공와우 이식 집단에서의 어머니의 시도 빈도가 건청 집단에 비해서 유의하게 높았다. 두 집단 모두 어머니가 공동주의를 개시할 때, 두 가지 이상의 단서를 함께 사용하는 통합적 단서의 사용 빈도가 다른 단서 유형에 비해서 유의하게 높았다. 공동주의 확립 성공률은 집단 간에 유의한 차이가 없었으나, 공동주의 시간비율 및 평균지속시간에서는 집단 간에 유의한 차이를 보였다. 마지막으로 인공와우이식 집단에서의 부모와 영유아 간의 공동주의 성공률, 시간비율, 평균지속시간이 영유아의 수용언어 점수와 표현언어 점수와 유의한 상관이 나타났다. 논의 및 결론: 본 연구를 통해, 인공와우이식 집단의 어머니가 영유아와의 공동주의 확립을 위해 많은 시도를 하고 있으며, 어머니가 사용하는 단서 사용 양상은 인공와우이식 집단과 건청 집단 간에 비슷한 것을 확인하였다. 인공와우이식 집단에서의 어머니와 영유아 간 공동주의 성공률은 건청 집단과 비슷하더라도 공동주의 시간비율과 평균지속시간이 건청 집단에 비해서 짧게 나타나서, 인공와우이식 집단의 공동 주의가 비효율적이라는 것을 알 수 있었다.

영문 초록

Objectives: This study aimed to identify the differences in the establishment of joint attention between the cochlear implant (CI) group and the typical hearing (TH) group during mother-child interactions and to explore the relationship between the joint attention variables and language development abilities in young children with CIs. Methods: Thirteen pairs of hearing mothers and children with CIs and seventeen pairs of mothers and children with TH participated in this study. Their interactions during a 20-minute free-play task were recorded using digital cameras. Results: The frequency of joint attention attempts in the CI group was significantly higher than in the TH group. There was no significant difference between the two groups in the success rate of joint attention; however, there was a considerable difference in the time ratio of joint attention and joint attention span. Children’s language scores were significantly correlated with the success rate of joint attention, the time ratio of joint attention, and joint attention span in the CI group. Conclusion: We observed the mothers’ efforts to establish joint attention with their children with CIs. These results suggest that children with CIs are more likely to have less time for joint attention during parent-child interactions. These findings can inform the development of parentcoaching programs in early auditory intervention for children with CIs.
