심근경색증 환자의 우울, 불확실성, 의료진과의 관계와 환자 역할행위 이행

국문 초록

최근 증가하고 있는 심근경색증 환자는 일생동안 재발과 합병증의 위험성과 함께 미래에 대한 불확실성에 따른 우울 등 여러 어려움을 가지고 살아가야 하며 수명연장을 위해 지켜야 할 생활 습관들을 꾸준히 이행하는 것이 중요하므로 의료진들의 지지가 필요하다. 또한 시간 경과됨에 따라 낮아지는 환자역할 행위를 지속시키기 위한 영향요인을 규명하는 것은 매우 중요하므로 본 연구는 심근경색증 환자의 우울, 불확실성, 의료진과의 관계와 환자 역할행위 이행과의 관계를 확인하기 위해 시도되었다. 연구방법은 서술적 상관관계 분석연구로 2014년 4월 1일부터 11월 28일까지 경기도에 거주하는 심근경색증 환자 112명을 조사하여 단계적 회귀분석을 이용하여 분석하였다. 연구결과 환자 역할행위 이행에는 교육수준, 경제 상태, 유병 기간, 재활교육 횟수, 입원 횟수에 따라 차이가 있었으며 우울, 불확실성, 의료진과의 관계와 유의한 상관관계가 있었다. 환자 역할행위 이행에 영향을 주는 유의한 변수로는 우울, 불확실성, 의료진과의 관계, 경제 상태, 재활교육 횟수이었으며 이 변수들은 환자 역할행위 이행을 41.9% 설명하였고 이 중 우울이 가장 영향력이 큰 변수였다. 따라서 심근경색증 환자들의 환자 역할행위 이행을 높이면서 지속시키기 위해서는 우선 중점적으로 우울과 불확실성을 감소시키고 의료진과의 관계 및 재활교육을 강화시키며, 경제상태로 인한 문제를 고려하여 프로그램을 개발, 적용시켜야 할 것이다.

영문 초록

Myocardial infarction patients comply with strict medical regimen to live with many complications, accept restrictions and continuously communicate with health care providers. Establishing influence factor that keeps compliance of health behavior is the most important. This research is to provide a basic data to develop a nursing intervention program and to identify factors which influence compliance of health behavior by recognizing a relation between depression, uncertainty and patient provider relationship. The study design was a cross sectional descriptive study and the data was collected on 1st April to 28th November 2014 with a structured questionnaire from 112 patients in Kyung-gi province. As a research finding, there were differences in educational background level, economic status, number of rehabilitation education, disease duration, number of admission in accordance with compliance of health behavior. Depression, uncertainty, patient provider relationship economic status, number of rehabilitation education were the significant factors and explained 41.9% of compliance of health behavior. Of all, depression was the most explicable variable. Thus, there should be further development of professional nursing strategies that can intervene compliance of health behavior by reducing depression considering these variables.
