노인의 자살 생각에 영향을 미치는 요인

국문 초록

본 연구는 자살생각 노인과 동일한 특성을 가진 비자살생각 노인을 성향점수 짝짓기(propensity score matching) 방법으로 추출하여 노인의 자살생각에 영향을 미치는 요인들을 규명함으로써 노인의 자살방지를 위한 간호중재의 기초자료를 활용하고자 시행되었다. 연구의 대상자는 2013년도 제 8차 한국복지패널조사에 참여한 노인 중 자살생각을 한 적이 있는 노인 247명과 성별, 연령, 경제상태를 사전조건으로 추출된 비자살생각 노인 247명이었다. 분석은 기술통계, 카이제곱검정, Mann-Whitney U test, 로지스틱 회귀분석을 이용하였다. 자살생각 노인과 비자살생각 노인의 배우자 유무, 종교유무, 동거가족 여부, 가족관계 만족도, 주관적 건강상태, 입원경험 유무, 우울 및 자아존중감 정도에서 차이가 있었다. 자살생각의 예측변인은 가족관계 만족도, 우울 및 자아존중감으로 나타났으며 이들 변인은 자살생각의 45.7%를 설명하였다. 이상의 결과를 통하여 노인의 자살의 위험성을 낮추기 위해 우울수준을 낮추고 자아존중감을 향상시키기 위한 중재뿐만 아니라 가족관계 개선방법도 함께 모색되어야 함을 알 수 있다.

영문 초록

The purpose of this study was to identify influencing factors in suicidal ideation in the elderly using a population-based survey data. This cross-sectional study used the 8th Korean welfare penal study collected in 2013. We have identified 247 self-reported older adults with the experience of suicidal ideation during the last year and selected 1:1 matched older adults with no experience of suicidal ideation by age, gender, and economic status. Socio- demographic characteristics with satisfaction in family relationship, physical characteristics including perceived health status, admission history, chronic disease, numbers of outpatient visits, and alcohol use, and psychological factors including depression and self-esteem were measured. Data were analyzed with descriptive statistics, chi-square test, Mann-Whitney U test, and logistic regression. The results of logistic regression analysis showed that satisfaction in family relationship, depression, and self-esteem were associated with suicidal ideation in the elderly (chi-square=207.40, p<.001, Nagelkerke R²=.46). Older adults are a vulnerable population at high risk of suicide. Systematic supporting system should be developed and continuously provided to control suicide in the elderly.
