노인의 구강건강관련 삶의 질에 영향을 미치는 요인

국문 초록

본 연구는 노인의 구강건강관련 삶의 질에 영향을 미치는 요인을 파악하기 위한 서술적 조사연구이다. 연구 대상자는 일 도시의 65세 이상의 노인 128명으로 식습관, 영양상태, 구강건조증이며 구강건강영향지수를 이용하여 구강건강관련 삶의 질을 측정하였다. 수집된 자료는 SPSS 21.0 프로그램을 이용하여 t-검정(t-test), 일원분산분석(ANOVA), 비모수 분산분석(Mann-Whitney U test, Kruskall-Wallis test), Pearson’s 상관계수(correlation coefficient), 다중회귀분석(multiple regression analysis)을 실시하였다. 본 연구변수들 간의 상관관계를 분석한 결과, 식습관은 영양상태(r=0.309, p<.001), 구강건강관련 삶의 질(r=0.174, p=.049)과는 순상관관계를 나타냈으며, 구강건조증(r=-0.302, p<.001)과는 역상관관계를 나타냈다. 영양상태는 구강건조증(r=-0.585, p<.001)과 역상관관계, 구강건강관련 삶의 질(r=0.558, p<.001)과는 순상관관계를 나타냈다. 구강건조증은 구강건강관련 삶의 질(r=-0.511, p<.001)과 역상관관계를 나타냈다. 구강건강관련 삶의 질에 영향을 미치는 요인은 틀니 유형, 주관적 건강상태, 주관적 구강건강상태, 영양상태, 구강건조증이 주요 요인으로 나타났으며, 구강건조증(beta=-0.347)이 가장 높은 영향요인으로 나타났다. 본 연구결과를 토대로 지역사회 보건의료 인력은 노인의 구강건강관련 삶의 질 향상을 위한 전략으로 일상생활에서의 구강건조증 및 치아상실을 예방하기 위한 구강관리방법 교육, 정기적인 구강검진을 통한 구강건강 유지를 위한 전략을 수립하여야 할 것이다.

영문 초록

This study is a descriptive research to investigate factors that influence the elderly s oral health related quality of life living in home. Research subjects were 128 elderly people aged 65 or over in a city; research tools were diet habits, nutritional status and xerostomia; oral health related quality of life were measured using oral health impact profile. t-test, ANOVA, post-hoc test were implemented by using Scheffe test, Mann-Whitney U test, Kruskall-Wallis test, post-hoc test were implemented by using Bonferroni correction, Pearson’s correlation coefficient and multiple regression analysis were used, using SPSS 21.0 program. Factors that influence oral health related quality of life are the types of dentures, subjective health status, subjective oral health status, nutritional status, xerostomia; the xerostomia was found to be the strongest influencing factor. It is needed to establish strategies for prevention of xerostomia for the elderly to have the better oral health-related quality of life, based on these research results.
