국내 A형간염 위험지역과 위험시기 탐색

국문 초록

A형 간염(hepatitis A)은 A형 간염 바이러스(hepatitis A virus)에 의하여 발생되는 간염이다. A형 간염은 대부분 소아 청년기에 감염되어 후유증이 심하지는 않지만 나이가 들어 감염이 되면 간이 치명적으로 손상되어 사망에 이르는 심각한 질병이다. 최근에 A형 간염 환자수가 지속적으로 증가하고 있어 사회적·경제적 부담이 점점 커지고 있으나 유병율과 감염경로 등에 관한 연구가 대부분이며 고위험 지역 탐색에 관한 연구는 미흡한 실정이다. 본 연구의 목적은 2011년 이후 2018년까지 국내에서 발생한 자료를 포아송 모형과 시공간 순열 모형을 사용한 스캔통계학으로 분석하여 A형 간염이 높게 발생한 고위험 지역과 시기를 추정하는 것이다. 분석결과 여성보다 남성에게 6:4 비율로 더 많이 발생하며, 환자의 85%는 20대~40대이다. 지리적으로 충청남도와 경기도의 서해안에 가까운 지역들이 고위험 클러스터로 나타났으며 도시 단일 지역으로 목포와 광주가 고위험 2차 클러스터에 포함되었다. 본 연구 결과에 근거한 국내 A형 간염의 고위험 클러스터에 관한 정보는 A형 간염의 예방 표본 감시 체계에 유용하게 활용될 수 있을 것으로 예상한다.

영문 초록

Hepatitis A is infected by hepatitis A virus. Although most of the infections in children and adolescents are not severely sequelae, it is a serious disease that causes fatal damage to the liver and death when the elderly are infected. Recently, the social and economic burden is increasing due to the continuous increase in the number of hepatitis A, but most of the studies on the prevalence rate and the path of infection so that the research to detect the high incidents of hepatitis A are insufficient. From 2011 to 2018, the data about the number of hepatitis A in Korea were analyzed by scan statistics in which Poisson model and space-time model were applied. As a result, hepatitis A occurs more in the male than in female at 6:4 ration and about 85% of patients are from 20s to 40s. It is also identified that the areas near the west coast of Chungcheongnam-do and Gyeonggi-do were high-risk clusters. Based on the results of this study, it is expected that the information on high risk clusters of hepatitis A in Korea will be useful for the preventive monitoring system of hepatitis A.
